Although rarely employed in usual clinical practices, in depth information about comorbid psychopathology can be obtained by using structured interviews to assess for DSM diagnoses including the structured clinical interview for DSM-IV Axis I (SCID-I) and the Structured Clinical Interview for DSM-IV Axis II (SCID-II).

Patients aged 18–45 years with the first episode of depression diagnosed by criteria set out in the Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition (DSM-IV) were eligible for our study. Two psychiatrists who received the training on the diagnostic criteria of DSM-IV before the study has assessed all subjects. And all the participants received a diagnostic interview based on the Structured Clinical Interview for DSM-IV at baseline and a 12-week follow-up. Patients had to meet the following inclusion criteria: (1) able to provide informed consent; (2) 24-item Hamilton depression scale (HAMD) score ≥21. All participants had to be under the care of an adult caregiver who monitored and recorded the intake of medication daily during the trial to assure medication adherence. The exclusion criteria are as follows: (1) suffering from other serious somatic diseases or comorbidities; (2) patients with serious nervous system disease; (3) patients met diagnostic criteria for mania (DSM, ICD); (4) patients by diagnostic standards of other mental illness; (5) patients who need to take benzodiazepine every day, and who currently need to be treated by electroconvulsive therapy or have received electroconvulsive therapy in the past 6 months; (6) pregnant women or lactating women; (7) patients with an apparent suicide attempt or suicidal behaviour; (8) any condition or medicines that may affect biomarkers (within 1 week of the screening period or during the whole trial period).

Dorsolateral prefrontal cortex (DLPFC; Brodmann area 9) brain tissue samples were obtained from the Maryland and Alabama Brain Collections. Two independent psychiatrists established DSM-IV diagnoses based on the review of available medical records, autopsy reports, and interviews with family using the Structured Clinical Interview for DSM-IV. All cases were obtained with consent from the next of kin with IRB approved protocols. Tissue was obtained from subjects diagnosed with major depressive disorder (MDD) and matched non-psychiatrically ill controls (CTL) and schizophrenia (SCZ) and matched non-psychiatrically ill controls (CTL) subjects (n = 20 per group). Subjects were matched for sex, age, post-mortem interval (PMI) and pH (Table 1). Thirteen subjects were common to both non-psychiatrically ill control groups.

Data were collected as part of the hospital’s Adult Outcomes Project (Allen et al., 2009), which was designed to incorporate standardized patient outcomes to track treatment progress. Within 72 hours of admission, participants completed standardized assessments on a hospital-wide web survey accessed via laptop computers. Participants also met in person with a research assistant to complete the Structured Clinical Interview for DSM-IV Disorders (First, Gibbon et al., 1997; First, Spitzer et al., 1997) which was used to establish psychiatric diagnoses (Table 2). Assessments were designed and incorporated into routine patient clinical care. Select measures were used as the predictor, outcome and mediating variables for this study. Less than 1% of patients declined participation. The collection and utilization of all data, including analysis and reporting of findings, was approved by the Baylor College of Medicine’s Institutional Review Board (IRB).
