Total Hip in a Day: Setup and Early Experiences in Outpatient Hip Surgery
Published in K. Mohan Iyer, Hip Joint in Adults: Advances and Developments, 2018
For bleeding prevention intravenous (IV) administration of 1 g tranexamic acid is administered. Additionally for pain, the patient receives 2 g of metamizole, 50 mg of dexketoprofen and 3 or 4 mg piritramide all intravenously. We also routinely administer a single shot of antibiotics (cefuroxime 1.5 g) intravenously (Table 18.1).

Electrospun polymer scaffolds were prepared from polycaprolactone (PCL), produced by Sigma-Aldrich, St. Louis, Missouri, United States, with Mn 80 000 gmol−1, Mw/Mn < 2 and density 1.145 g/mL at 25 °C. The polycaprolactone was dissolved in solution of glacial acetic acid and acetone in ratio of 8:2, and was homogenized for 24 h with a magnetic stirrer at temperature of 50 °C. Scaffolds were prepared from neat PCL and PCL/antibiotic CFU® solutions, with two different concentrations, 14% PCL + 15 wt% CFU® and 20 wt% CFU®. CFU® antibiotic (Astro Pharma, Vienna, Austria) has active substance cefuroxime sodium with the chemical structure as shown in Figure 1. Fibers were electrospun on standard electrospinning device NT-ESS-300, NTSEE. Co. Ltd., South Korea, with modified collectors. Electrospinning was performed at voltage of 17 kV, with a jet speed of 1 mL/h, while the fibrous scaffolds were collected through 2 h. Polymer solution feed was 2 mL, and the distance between the needle and the collector was maintained at 18 cm. Polymer fibers were deposited on 3D printed collectors with six different configurations. Three ribbed and three mesh collectors were designed with 123D Design and CATIA V5 and then printed with a 3D printer Formlabs. The dimensions of the six collectors are given in Table 1. The design of the mesh collector is shown in Figure 2.

Materials used in this work were: polycaprolactone (PCL) with Mn = 80,000 (Sigma Aldrich) and antibiotic cefuroxime (CFU) (Astro Pharm). Polymer solution of 18% was prepared by dissolving the polymer in glacial acetic acid and acetone with the volume ratio of 8:2. The concentration of the antibiotic was 5 and 10 wt% in regard to the polymer mass. The blend solution was prepared by dissolving the polymer with the addition of the antibiotic with constant stirring and heating at 50 °C.

This test was carried out against a Gram-negative bacteria Escherichia coli and a Gram-positive bacterium was Bacillus cereus. The aqueous leaf extract of C. aconitifolius and ZnO NPs were tested using the well diffusion method according to Ahmed et al. (43). Cefuroxime was the standard antibiotic used as the positive control for the bacteria test organisms. The diameter zones of inhibition were determined.
