In advance of our meeting, Sam sent me a copy of the neuropsychological assessment report, written by a neuropsychologist from her HMO. Her diagnosis: axis I, 294.9 cognitive disorder not otherwise specified (NOS), associated with 314.01, attention-deficit/hyperactivity disorder, predominantly inattentive type; and, axis II 301.9, personality disorder NOS with avoidant, and dysthymic features. According to the Diagnostic and Statistical Manual of Mental Disorders (2000) 294.9, cognitive disorder NOS may include an “impairment in cognitive functioning as evidenced by neuropsychological testing or quantified clinical assessment” (American Psychiatric Association, 2000, p. 98).

According to DSM-5 (American Psychiatric Association 2013), a persistent pattern of inattention that interferes with functioning or development is characterised as Attention Deficit Hyperactivity Disorder – Predominantly Inattentive Presentation (ADD). Further requirements are that six or more of the following symptoms (listed here in short form) have persisted for at least 6 months to a degree inconsistent with developmental level and negatively impact directly on social or academic/occupational activities, with several symptoms present before age 12 years and present in two or more settings:Often fails to give close attention to details or makes careless mistakes;Often has difficulty sustaining attention in tasks or activities;Often does not seem to listen when spoken to directly;Often does not follow through on instructions and fails to finish schoolwork, chores, or duties in the workplace;Often has difficulty organising tasks and activities;Often avoids, dislikes or is reluctant to engage in tasks requiring sustained mental effort;Often loses things needed for tasks or activities;Is often easily distracted by extraneous stimuli;Is often forgetful in daily activities.
