Keratolytics, astringents, and defatting agents are largely over-the-counter preparations, and are used to treat acne and related dermatologic conditions (Box 13.3). No human reproduction studies for any of these agents are published, nor are animal teratology data. Topical use of benzoyl peroxide, resorcinol, and salicylic acid may result in systemic absorption, but no cases of adverse fetal effects are documented related to the topical route of delivery. Salicylates are discussed in detail in Chapter 8, Analgesics during pregnancy. Manufacturer data on salicylic acid was reported to be teratogenic in animals when used in large doses, several times that used in humans.

Of 8 patients with contact allergy to methylol phenols present in phenolformaldehyde resins, 4 (50%) co-reacted to salicylaldehyde (7). Among 17 patients sensitised to resorcinol by application of a wart remover, 2 positive reactions to salicylaldehyde were observed (8).
Resorcinol

Resorcinol is a 1,3-isomer (or meta-isomer) of benzenediol that formerly had widespread pharmaceutical use in anti-acne preparations, anti-wart ointments, antipsoriatic creams, exfoliative preparations, suppositories, eye drops and Castellani’s paint, because of its (alleged) antiseptic, itch relieving, keratolytic and antifungal properties. Currently, it is used as a coupler in hair dyes and in many industrial applications. Consumer products that may contain resorcinol include hair dyes, adhesives and sealants, building/construction materials, wood and engineered wood products, electrical and electronic products, fabric, textile, and leather products, paints and coatings, personal care products, plastic and rubber products, and rubber tires (36,37). Resorcinol may also be present in some patch test marking inks (23).

Two novel scaffolds of diaryl ether NNRTIs with phenacyl moiety were designed in this study. With the aid of molecular modelling several modifications of the core structures were prepared. Using molecular docking to several crystallographic structures and thorough conformational search of ligands, it was possible to obtain quite accurate predictions of structure–activity relationship. All synthesised compounds showed inhibitory activity against wild-type HIV-1 RT. In general, resorcinol-based compounds possessed better activities than catechol-based compounds, and are more promising candidates for further development. One of the compounds, 7e, was found to be a very potent NNRTI in enzymatic assay, and is predicted to display novel interactions with the RT. The presented compounds were designed to possess a good aqueous solubility, which was achieved in all cases. Given those encouraging results, the inhibitors were subjected to biological evaluation of their efficacy against HIV infection in vitro. 7e proved to be a potent anti-viral in SupT1 and CD4+ T cell infectivity assays. These results show our design could deliver highly water-soluble NNRTIs, at least one compound displays potent antiviral activity in infection assays in vitro.
Intense pulsed light treatment for patients with hidradenitis suppurativa: beware treatment with resorcinol

Patients had a mean age of 39.2 (S.D 10.0) and a mean disease duration of 13.2 (S.D 10.9). A Hurley score of I was recorded for three patients, II for 21 patients and III for one patient. Seventeen patients had Fitzpatrick skin type 2, six had skin type 3 and two had skin type 4. Patients were treated in one or more regions. A total of 22 patients were treated in the pubic region, 12 in the groin, six in the axilla, three at the labia major and one on the buttocks and one on the inner thighs. A total of 13/25 patients reported a reduction in disease activity and 17/25 reported an effect on hair, and 7/25 reported adverse effects of the treatment (Table 1). Hair reduction co-occurred with disease activity reduction in all cases (p = .001), but the effect on hair and on HS did not co-occur with adverse effects (p = .374 and p = 1.0, respectively). The effect on HS, the effect on hair and the occurrence of adverse effects were tested against the concomitant treatments the patients received. Azeleic acid cream (20% azeleic acid, Skinoren®, Bayer AG) was associated with a reduced likelihood of effect of IPL treatment for hair reduction (Fisher’s exact test, p = .038, but not on HS = 0.357). All patients that experienced adverse effects were treated with resorcinol (proprietary cream, 15% resorcinol in a mix of 30% glycerol (85%) and 55% soft yellow paraffin) (Fisher’s exact test, p = .020). Adverse effects included severe erythema and irritation of the area after shaving for the procedure (n = 4), minor infection (n = 1) and minor burn (n = 1), worsening of HS the day after treatment (n = 1).

HS is difficult to manage. Antibiotics, topical, and systemic are the mainstay treatment for mild (Hurley I and mild Hurley II) HS, although only two randomized controlled trials have been conducted to determine the efficacy of antibiotics (9,10). Topical resorcinol is an alternative treatment. Its use is limited because it may be absorbed percutaneously and discoloring effect. It cannot be used during pregnancy (8,11–13). TNF-α inhibitors are effective for HS but are costly. Surgery is the only curative therapy for HS, serious wound care and cosmetic problems may accompany these procedures.
