Solitary cardiac fibroma appears to be a rare but valid part of the syndrome;38 calcification may be present on occasion. The relationship of sundry other neoplasms to the syndrome (e.g., meningioma, craniopharyngioma, embryonal rhabdomyosarcoma, leiomyoma, fibrosarcoma, neurofibroma) is unclear.25,39

Benign tumors of fibrous tissue included in this group are cardiac fibroma, solitary fibrous tumor of the pericardium, benign fibrous histiocytoma and inflammatory pseudotumor. Amongst these rare tumors cardiac fibroma is the most common tumor of childhood after rhabdomyoma. Cardiac fibroma is a benign congenital tumor occurring as a discrete mass in the ventricular septum, left ventricular free wall, right ventricle and atria in that order. Microscopically, spindle shaped proliferating fibroblasts with variable amount of collagen is seen. Inflammatory cell infiltration consisting of lymphocytes and histiocytes is often encountered. The treatment is surgical excision (Figs 12.28 and 12.29).

Transthoracic echocardiography demonstrated a mass in the left ventricular myocardium (Figure 1(A)). Cardiac magnetic resonance imaging (MRI) confirmed the presence of an intramyocardial mass involving the basal anterolateral left ventricular wall in the region of the left ventricular outflow tract and extending to the left ventricular cavity which was intrinsically T1 and double inversion recovery T2-weighted image relative to myocardium (Figure 1(B,C)). Additionally, there was low early enhancement and avid delayed enhancement (Figure 1(D)). The constellation of imaging findings was most consistent with a benign cardiac fibroma. Near-total resection was performed, and histologic analysis demonstrated bland spindle cells with an abundance of collagen and elastin fibres (Figure 1(E)), confirming the diagnosis of cardiac fibroma.
