In rare circumstances, symptoms can start in the premenarchal period if the hemivagina has a small communication between the two vaginas, as described earlier [42]. Such patients can present with chronic leukorrhea as a result of accumulation of vaginal discharge above the hemivagina [39]. However, if there is no opening in the hemivagina, this may lead to swelling and discomfort as a result of fluid accumulation, resulting in hydrocolpos. In such cases, prepubertal patients may present with a bulging vaginal mass and vaginal discomfort. If such presentation occurs in the neonatal period, the mass and symptoms may subside spontaneously in a few months after cessation of exposure to maternal estrogen after birth [47, 48].

Leucorrhoea is caused by bacterial or fungal infections in the mucous membranes of the vagina. Therefore, an effective medicine against it should have bactericidal and fungicidal activity or should be able, especially in the case of a fungal infection, to lower the pH of the mucous membranes (Spence and Melville 2007). Chloroform, methanol, aqueous and ethanol extracts of the stem bark of S. indica were investigated for their antibacterial and antifungal activities against standard strains of the bacteria Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Bacillus cereus, Klebsiella pneumoniae, Proteus mirabilis, Salmonella typhimurium, Streptococcus pneumoniae and the fungi Candida albicans and Cryptococcus albidus. Methanol and aqueous extracts exhibited antimicrobial activity with MIC ranging from 0.5–2% and 1–3% respectively. Methanol extract exhibited the strongest activity against both bacteria and fungi (Sainath et al. 2009).

Leukorrhea (leuko- = white) is characterized by a distressing, often whitish discharge from the genital tract. Also simply called vaginal discharge, leukorrhea is a symptom rather than a disease, and the term should only be applied to an abnormal discharge. Common causes include chronic cervicitis, cervical eversion (turning inside out), and infections with organisms such as Trichomonas vaginalis, Candida, and Haemophilus vaginalis. If the discharge is due to inflammation of the vagina, it is called vaginitis.

All patients underwent gynecological TA and TV US to determine the number, location, size, shape, FIGO classification, and blood supply of the myoma, as well as its relationships with the surrounding structures. The volume of the myoma was calculated using the following equation: myoma volume (cm3) = upper and lower diameter (cm) × left and right diameter (cm) × anteroposterior diameter (cm) × 0.523. Routine blood tests, coagulation function, routine leucorrhea examinations, screening tests for cervical cancer, pelvic enhanced MRI and electrocardiography were completed. Patients were instructed to refrain from consuming solids and liquids for 6 h before surgery. Placement of an indwelling catheter and bowel preparation were undertaken when necessary. Wet vaginal gauze was applied during the ablation of lesions close to the endometrium in the TA group.

Vaginal atrophy causes several symptoms, such as vulvovaginal dryness, decreased vaginal lubrication, dyspareunia, vulvar or vaginal bleeding, decreased libido, itching and leukorrhea [31]. According to our results, in the review carried out by Dizavandi et al. [32] topical isoflavones presented better outcomes on vaginal atrophy. It was found that the administration of 80 mg of red clover oil had a significant effect on superficial (p < 0.005), intermediate (p < 0.005) and parabasal cells, and vaginal dryness (p < 0.005), compared to the placebo. In addition, genistein had a more prominent effect on the genital score. The severity of dyspareunia decreased by 27%.

Eight weeks of treatment with vaginal E3 (500 μg per ovule) induced a significant reduction of all subjective GSM symptoms (dryness, irritation, dyspareunia, chronic vaginal leukorrhea) up to 6 months of follow-up. However, the effect of vaginal E3 was diminished at 12-month follow-up and at 18-month follow-up. Some parameters were even significantly worse than before treatment (dryness, irritation, chronic vaginal leukorrhea) [12]. In contrast, vaginal E3 ultralow dose (50 μg/application) had a shorter long-term effect, as vaginal dryness and dysuria had significantly worsened 3 months after treatment cessation [11].
