Other sequelae include: ectopic pregnancy; pelvic inflammatory disease (PID), and tubo-ovarian abscess. The perihepatitis caused by Chlamydia is called Fitz-Hugh Curtis syndrome. Cervical cancer is associated with human papilloma virus. Ovarian cancer, which is associated with infertility, may also have an association with PID/ Chlamydia infection; however, as yet there is no hard evidence proving this.

Perihepatitis is characterized by right upper quadrant pain, fever, and local tenderness. Typically, the patient is a young sexually active female with a recent history of genital tract infection or pelvic inflammatory disease. However, the syndrome has also been described in males.131,132 The condition is frequently misdiagnosed as acute cholecystitis, and may also be confused with pleurisy, pneumonia, pulmonary embolism, and rarely, perforation of an abdominal viscus. The etiologic agents are Neisseria gonorrheae and sometimes, Chlamydia trachomatis. The diagnosis may be confirmed by microbiologic analysis of swabs from the cervix, urethra, rectum, and throat. Antichlamydial antibodies should be sought in the blood and urogenital discharge. Laparoscopy may detect “violin string” adhesions between the anterior surface of the liver and the adjacent abdomen and/or white fibrous plaques and tiny hemorrhagic spots on the liver.

There have been reported complications of endocarditis, meningitis, perihepatitis and osteomyelitis with DGI. Dissemination of infection is reported in 0.5–3% of cases [3]. As per the retrospective study by belkacem et al., there has been an increase in the incidence of DGI in recent years with joint involvement being seen more frequently. Osteomyelitis is usually seen as a complication following septic arthritis. Cases of isolated osteomyelitis without septic arthritis have been reported as well [4–8]. It is important that physicians be aware of potential complications so that it is recognized, and appropriate therapy is promptly instituted. Complications have mostly been reported when the diagnosis is delayed [9]. Our patient had a delay in diagnosis, initially diagnosed with pseudogout, likely due to incomplete history concerning for high-risk sexual behavior that led to missed diagnosis of gonococcal arthritis. He also received prednisone, which increased the risk for dissemination of infection. This rare sequela of disseminated infection can lead to permanent injury if not caught early. Taking a good sexual history is the key to making an accurate diagnosis and preventing delay of correct diagnosis. Physicians should be aware of clinical presentations of DGI, keep high index of suspicion based on high-risk sexual behavior history and start treatment as soon as possible.

Rare clinical manifestations that affect both males and females as a result of a C. trachomatis infection include conjunctivitis, respiratory and gastrointestinal infections, reactive arthritis, and perihepatitis [45,56,57]. In countries with poor sanitary conditions trachoma is the main cause of preventable blindness [3]. Lymphogranuloma venereum (LGV) infections can result in highly debilitating chronic diseases with bubo formation, fistulas, fibrosis, and rectal stenosis. Among MSM, LGV infections are on the rise [58].
