Antimicrobials during Pregnancy

Several anthelmintics are available to treated infested women, although it is usually not necessary to treat helminth infections during pregnancy. Both mebendazole (Vermox) and thiabendazole (Mintezol) are effective for a variety of helminths, including pinworms (enterobiasis), whipworm (trichuriasis), roundworm (ascariasis), and hookworm (uncinariasis). According to their manufacturer, none of these drugs was teratogenic in laboratory animals, although there are no adequate human reproduction studies.

This is the most commonly used benzimidazole derivative. It is very slightly absorbed from the intestinal tract. This anthelmintic shows a wide spectrum of activity, being effective against T. trichiura, hookworms, A. lumbricoides, and E. vermicularis. The usual dose is 100 mg twice a day for 3 d, both in children and adults.19,20 A single dose of 100 to 200 mg is sufficient for enterobiasis.21 There is general agreement on the lack of toxic effects and on the absence of side effects. In a small number of cases, Ascaris has been seen to migrate to the mouth in children heavily infected by this parasite who were under treatment with mebendazole. With regard to teratogenicity, it was found that mebendazole, when given during the period of organogenesis in rodents, induces abnormalities in the fetuses, consisting mainly of some skeletal abnormalities in the ribs and tail. Neither teratogenic or embryotoxic effects were observed in dogs, sheep, or horses. As a precaution, this drug should not be used during the first months of pregnancy.

Also known as threadworms, enterobiasis is particularly common in children. The female lays eggs at night, therefore causing intense itching. Ingestion or inhalation of eggs causes infection of the intestines, which manifests as anal pruritis or very occasionally abdominal pain.

Lastly, it must be highlighted that IVM has a wide range of biological properties, indicating it as a potential candidate for drug repositioning, considering its well-established off-label use in parasitic diseases such as pediculosis, demodicosis, blepharitis, filariasis, gnathostomiasis, cutaneous larva migrans, trichuriasis, and enterobiasis (Crump 2017); or its off-label use under investigation, regarding its anti-inflammatory (Yan et al. 2011; Aryannejad et al. 2022), anti-tumoural (Wang et al. 2016; Sharmeen et al. 2010), antibacterial (Ashraf et al. 2018; Paradies et al. 2019), and antiviral properties (Croci et al. 2016; Surnar et al. 2020).
