Dandruff or seborrheic dermatitis is treated with antiseborrheic agents (Box 13.4). The mechanism of action is unknown. No animal or human reproduction studies are published. It is very unlikely that these agents have any effect on prenatal development because they are not absorbed systemically. Coal tar and salicylic acid are often used in combination with other agents, such as sulfa, and in combination with one another to treat seborrhea and seborrheic dermatitis.
Tioxolone

Tioxolone is a 1,3-benzoxathiole derivative having a hydroxy substituent at the 6-position. It has a role as an antiseborrheic agent in anti-dandruff shampoo and is or probably was also used to treat acne vulgaris and psoriasis, often in combination products (1,4).

With respect to the plants that are used in Traditional Medicine, different groups of researchers have made important efforts to gather the ethnopharmacological information of Argentina. Regional ethnobotanical studies have been mainly carried out and these were taken into account to classify the first group of plants that correspond to those that present some use related to fungal infections (Ruiz Leal 1972, Bustos et al. 1996, Del Vitto et al. 1997a, b, Roig 2001, Scarpa 2004, Goleniowski et al. 2006). The following uses were considered: vulnerary, antiseptic, antidandruff, antialopecic, antiseborrheic, antipruritic, disinfectant, to heal wounds, ulcers, psoriasis, leucorrhea and venereal diseases. These are all ailments that may be caused by some type of fungus (Petenatti et al. 2008).

A randomized comparative trial comparing alternate daily use of 10 mg isotretinoin versus topical anti-seborrheic treatment in moderate to severe seborrhea and seborrheic dermatitis (N = 45) over 6 months, demonstrated a significantly greater reduction in scalp sebum secretion (p = .004) in the isotretinoin arm after 3 months (47). Patients were not followed-up beyond 3 months and the dosage was not weight-dependent. Tagliolatto et al. treated 20 patients with sebaceous hyperplasia with isotretinoin (1 mg/kg per day) for 2 months; the average number of skin lesions decreased from 24 to 2 per patient (p < .05) (59). Isotretinoin is beneficial in sebaceous hyperplasia and sebaceous adenoma (7), cyclosporine-induced sebaceous hyperplasia in renal transplant (15,23), and diffuse familial sebaceous hyperplasia (14).

Psorisdin Shampoo® is specifically designed for scalp hygiene in patients suffering from thick scales, redness and flaky scalp. Its formulation contains urea, glycolic acid, salicylic acid, ichthyol pale and laureth 9 that contribute all together to an immediate and long-lasting anti-inflammatory, anti-itching, anti-sebum, keratolytic and antimicrobial effects on the scalp, simultaneously taking care of the hair and scalp health and beauty. Urea ingredient has antifungal, antimicrobial qualities and a high ability to act as a moisturizer for dry and flaky scalp. Salicylic acid provides to the shampoo a keratolytic effect, as it loosens and softens scalps scales, making them easier to remove. Zinc pyrithione (ZPT) is a 1:2 compound of a Zinc atom and bonding the membrane-permeable ionophore pyrithione. The Food and Drug Administration (FDA) regards it as a worldwide microbicidal agent and generally applied in medicine and cosmetology, including anti-dandruff shampoos because of its antibacterial, antifungal, or antiseborrheic activity [13–15]. Besides, ZPT has been found that it can act as a Zinc ionophore to help transport Zinc across biological membranes and increase the intracellular Zinc ion concentration [16].
