Thickening and calcification of the mitral valve leaflets, which are often associated with mitral stenosis (Figure 6.7), and calcification at the mitral valve annulus (Figure 6.8) can be described. In patients with mitral valve prolapse, the typical systolic displacement of the mitral valve leaflets can be demonstrated (Figure 6.9). CT has recently been described in the assessment of mitral annular disjunction.69

MDCT plays a complementary role in the adjudication of degenerative mitral valve disease (DMVD) and functional mitral regurgitation (FMR). The two groups have distinct pathophysiology resulting in different mitral annular and landing zone anatomy. DMVD is due to fibroelastic deficiency and myxomatous degeneration, leading to chordal elongation, valvular thickening and prolapse of the leaflets. The insertion of posterior mitral valve leaflet and annulus is commonly displaced to the left atrium, known as mitral annular disjunction (Figure 1).

Objective: Mitral annular disjunction (MAD) has been associated with myxomatous mitral valve disease (MMVD) but functional analyses of the mitral annulus (MA) and the entire left atrio-ventricular junction (LAVJ) are lacking from high-resolution imaging to understand MAD physiopathology. Using multiphase cardiac computed tomography imaging (4D-MCCTI), we aimed at characterizing the morphological changes of MA and LAVJ throughout the cardiac cycle.
