Ulegyria is a cortical scar due to ischemic injury in full-term infants. Minimal perfusion at the deeper portions of the gyri leads to its shrinkage and atrophy in comparison with the apical part of the gyri, giving it a characteristic mushroom shape. Other types of cortical and glial scars may result from infective or traumatic injury.

After systemically and comprehensively searching the dataset of Tiantan Epilepsy Center between January 2015 and June 2017, fourteen patients who received reoperation after implantation of SEEG were prospectively enrolled in the study (Figure 1). Table 1 displays the details of the patient demographics. The median age of 1st operation is 16.0 years (range from 10.0 to 25.5), the median age of 2nd operation is 23.5 years (range from 14.8 to 29.5); two patients were females and 14 were male. FCD were the most common surgical pathological diagnosis (8/14), followed by the ulegyria (14.3%) resulting from previous surgery. Prior to the presurgical reevaluation, none of the patients had reached a seizure-free status (ILAE 1), two (14.3%) patients were ILAE 2, three (21.4%) were ILAE 3, five (35.7%) were ILAE 4, three (21.4%) were ILAE 5, and one (7.1%) patient was ILAE 6. The different semiology of each patient between two presurgical evaluation has shown, and it is not difficult to find that there is no or moderate change after previous epileptic surgery in 10 patients, which usually indicates that the EZ is not completely eliminated before. Reevaluation data enables to provide useful information to plan the implantation scheme. Integration of presurgical assessment data, including semiology, interictal/ictal EEG, MRI, PET-MRI, is carefully collected and analyzed to localize the seizure focus and select eligible candidacy.
