Urethral discharge is usually associated with dysuria and is usually due to chlamydial infection or gonorrhoea. A sexual history should be obtained, and the patient should be referred to a clinic for sexually transmitted diseases.

Characteristics of a study population was presented in Table 1. In the total of 349 heterosexual men diagnosed with symptomatic urethritis, we found that 11.2% reported using antibiotics before presenting at our clinic. Urethral discharge was the predominant symptom in most cases (88.8%). There were 53.3% unmarried men. The number of men who reported having sex with only one romantic partner, only one unstable partner, multiple sex partners, and a female sex worker was 139 (39.8%), 65 (18.6%), 57 (16.3%), and 88 (25.3%), respectively. There was a significant difference in the type of sexual partner regarding marital status, with p < 0.01. Married men were more likely to have sex with multiple sex partners (27.6 vs. 6.5%) or with a female sex worker (36.2 vs. 15.6%) than unmarried men, while the latter appeared to be more faithful, with 47.3 vs. 31.3% reporting only one romantic partner in the last three months.

A 67-year-old male presented to the Department of Urology, San Martino Hospital (Genova) in December 2017 with high temperature and purulent urethral discharge. He was cigarette smoker and suffered from chronic obstructive pulmonary disease, but apart from that the patient’s medical history was unremarkable. The patient did not provide any history of urethral trauma or sexually transmitted disease. In July 2017, a suprapubic cystostomy had been placed for the management of obstructive voiding symptoms, after failed urethral catheterization in another Department. A urethral stricture had been suspected. In September 2017, the patient had been admitted because of penoscrotal abscess extending to surrounding subcutaneous tissue. A perineal urethrostomy had been performed after several unsuccessful attempts of conservative treatment. A malignant disease was not rule out.
Adolescents and Young People’s Sexual and Reproductive Health in Iran: A Conceptual Review

Sexuality before marriage in such a cultural context has important adverse outcomes because young people do not receive any education on relationships and sexual health. Women who live in a conservative society with a strong endorsement of virginity are more vulnerable to violence, abuse, and honor-related killing if they get involved in premarital sex (Cinthio, 2015), as well as an increased rate of sexually transmitted infections (STIs). A cross-sectional study in Iran showed that the prevalence of male urethral discharge was 0.40% and for genital ulcers in women and men was 3.68% and 0.16%, respectively; these conditions may be symptoms of STIs. The incidence of gonorrhea, Chlamydia, and syphilis per 1,000 women was 2.44, 5.02, and 0.04, respectively, and per 1,000 men was 0.43, 0.82, and 0.005 (Nasirian et al., 2015). A study among youth in Shiraz showed that about 7% of sexually experienced youth (n = 528) reported a diagnosis of gonorrhea and only about 5% received treatment (Honarvar et al., 2015).
