Research into alternatives to standard inpatient acute hospital wards continues. This includes a systematic review of the Soteria paradigm, evaluating the empirical evidence of the North American version of Soteria and a later Swiss version.32 This was followed by a systematic review of residential alternatives to hospital-based acute inpatient care in the UK.33 Johnson et al.34 identified 131 so-called alternative services in England. However, in 109 of these, little identifiable difference was found in terms of the therapeutic approach between the alternative and standard services. This means that, although the services were located away from traditional hospital settings, the philosophy of care did not appear to be significantly different and that a relationship-centred approach was not necessarily adopted. Despite this, people using the services reported that they found alternative services to be less coercive and suggested that it was possible to build safer relationships within those environments. Alternative services were also cheaper to run and resulted in a shorter length of stay.34

Further, the costs of these alternative treatments are quite varied. For example, the hostels described in the Brook study cost a total of $410 per month for the whole operation. The studies range all the way up to that of Soteria, described by Mosher, Menn, and Matthews (1975), in which the cost of a patient for the alternative care was essentially the same as the cost for those in the inpatient unit. In no case did the cost of alternative care exceed that of inpatient care (but see Weisbrod et al., 1980).

Various possible models of implementation in different countries have been outlined. The representation agreement in Canada suggests appointing guardians chosen by the individual to support him or her in decision making. The PO scheme, introduced before the CRPD, offers the service of a PO, who actively engages with the individual in need of support over a long period in order to communicate the person’s decisions. The open dialogue model involves family, social network, and patient dialogue in all treatment decisions. The Soteria project, also dating well before the CRPD, encourages staff to spend more time with patients in order to support them in decision making and provide human support. AD were cited as an interesting means for patients to state their preferences for treatment when in a period of stability to be applied during times of crisis. Peer advocacy and support uses the experience of peers to support persons with mental health disorders in order to advocate for them in times of crisis. In South Australia, individuals can choose a person they trust to write “support agreements” under the Office of the Public Advocate. The facilitated decision-making approach is more intensive and not directly an SDM model, as it is used for persons who have significant disabilities and difficulties in communicating their decisions. The ASK ME approach proposes practically putting into action the SDM process, and two case examples were described.

The long-term follow-up study to STELLAR and SOTERIA,is ongoing. SOTERIA is an open-label, single-group trial, primarily to determine any long-term adverse events. Efficacy measures are secondary outcomes in SOTERIA [9].
