Specialty pharmacy is defined as the service created to manage the handling and service requirements of specialty pharmaceuticals, including dispensing, distribution, reimbursement, case management, and other services specific to patients with rare and/or chronic diseases. Specialty pharmacy provides two key deliverables: (1) a mechanism to manage the cost of specialty pharmaceuticals for the patient; and (2) an opportunity to save money for the benefit sponsor as compared to traditional models in which products are delivered through less efficient means, primarily hospitals or physicians' offices. As a side note, specialty pharmacies need separate licensing to be called a specialty pharmacy.
Adherence to cystic fibrosis transmembrane conductance regulator (CFTR) modulators: analysis of a national specialty pharmacy database

This retrospective observational cohort study utilized pharmacy fill data from a national specialty pharmacy to assess the medication adherence of CFTR modulators. The database includes patients from different US geographical regions who are on commercial and public health insurance plans. The national specialty pharmacy provides varying clinical pharmacy services such as refilling and shipping specialty medications, counseling patients regarding medication adherence, and management of adverse events associated with the specialty medications. Fill data for patients using CFTR modulators from September 2017 to August 2018 was utilized. Patients with one fill date along with the ones who had switched CFTR modulators during this period were excluded from the study. No identifiable protected health information was extracted or accessed during the study and a unique identifier was assigned to each patient in the database. The study was approved by the University’s institutional review board and the legal team of the national specialty pharmacy.

Specialty Pharmacy’s comprehensive, coordinated, and patient centric model is integral to patients achieving a successful treatment outcome. Specialty pharmacies connect patients who are severely ill with the medications prescribed for their conditions. However, the provision of the drug itself is simply transactional. The value of a specialty pharmacy is directly related to the patient management and product support services provided and patient interaction necessary to maximize the patient’s chances of achieving a successful outcome. Specialty pharmacy was perfectly positioned to respond to the challenges posed by the COVID-19 pandemic, able to support shelter in place requirements, and effectively manage patients remotely due to their established business model, technological infrastructure, and patient management and fulfillment capabilities. As we learn more about this novel coronavirus and new formulations of specialty drugs are explored as potential treatment options, NASP members will continue to play a vital role in the management of our most vulnerable patients.
