Prediabetes can be reversed with lifestyle changes and is commonly diagnosed in primary care when blood tests are requested as part of the NHS Health Check or as part of the annual review of a patient for a long-term condition such as hypertension. The NHS Health Check is provided for adults in England aged between 40 and 74, who do not have pre-existing conditions such as hypertension, stroke, atrial fibrillation, diabetes or heart failure. The GP surgery sends an invitation to people in this age group by letter or text for a free health check. Registered patients may contact the surgery to book their health check.

NHS health checks were performed on-site. The way in which these health checks were carried out required modification of the traditional model – the outreach nurse used a more conversational approach in light of literacy levels and their related challenges.

National programmes in the UK include screening for breast cancer, bowel cancer, cervical cancer, abdominal aortic aneurysm, chlamydia, plus antenatal and neonatal conditions. There is currently some debate concerning whether programmes should be established for diseases such as prostate cancer. The NHS Health Check Programme is offered to people aged between 40 and 74, aimed at helping to prevent heart disease, stroke, kidney disease, diabetes and dementia. Other screening takes place in various settings, such as eye tests for certain patients with disabilities and tests for diabetes.

The English National Health Service (NHS) Health Checks program started in 2009, aiming to reduce CVD incidence6. Its primary goal was to provide a routine structured clinical assessment and management for adults aged 40–74 years without pre-existing diabetes or CVD6. The NHS Health Check included review of CVD risks, behavioral change support and treatment of newly identified risk factors or comorbidities through integration with routine clinical provision in general practice6. Four years after its implementation, an analysis of the performance of NHS health check reports demonstrated that attendance quadrupled (from 5.8% in 2010 to 30.1% in 2012)6. After the health check, an increase in prescription with statins was noticed in high-risk patients, whereas a high rate of comorbidities and CVD risk factors were identified in the attendees6.

A close link between general practices and community-based lifestyle services is crucial for effective CMD prevention [4]. More than half of the index practices fall short in offering adequate lifestyle support services for weight management and/or exercise programs within their practice. This is worrisome since almost half of all index practices also lack an overview of available community-based lifestyle support services. These findings are more or less consistent with the study of Wyers et al. [10] who found that 62% of the respondents (i.e. GPs and healthcare professionals) were not informed about community-based lifestyle interventions. A qualitative evaluation of the National Health Services (NHS) Health check in the UK revealed the same lack of knowledge among caregivers [28].
