Management: Patients with ichthyosis vulgaris or other conditions associated with having “dry” or “sensitive” skin should be diligent about establishing good skin care routines. This means showering or bathing only once a day, for 5–10 minutes, with lukewarm water. This can be challenging in the summertime when patients are often taking multiple showers a day. Bath products should be fragrance free and designed for sensitive skin. Harsh soaps should be avoided. After the bath or shower, patients should pat dry with a towel, as opposed to harsh scrubbing or rubbing of the skin, and apply moisturizer right away. In general, patients should try to apply moisturizers twice a day. The “greasier” moisturizers, such as petrolatum, are typically the most effective, but the “best” are the ones that patients will actually comply with using. Creams can be lighter and easier to apply in the summer. Lotions can be more comfortable for areas with lots of terminal hairs, such as the chest of men. If the patient lives in centrally heated rooms, then humidifiers can raise the relative humidity. There are some current data supporting the use of biologic agents in the setting of ichthyosis, specifically secukinumab, which has been described as having beneficial effects on symptoms and appearance.

In clinical scores of TCS and VDS, we observed greater improvement in treatment group than control group for legs, but not observed for arms. Legs are more often affected by xerosis and the level of dryness is more severe than arms, thus the improvement may be more pronounced in legs (15). Moisturizer that is properly formulated is the mainstay of dry skin treatment (16). Besides moisturizer, mild cleanser is important in alleviating dry skin given that harsh soaps containing detergents strip away natural emollients in the skin, causing further drying and irritation (17). This likely explains the modest improvement that we observed in the control group who only used a gentle cleanser. Therefore, establishing a standardized skin care regimen is important and these benefits have been observed when compared to routine skin care on dry skin in nursing home residents (18). Although little is known regarding the efficacy of different types of skin care products because head-to-head comparisons are lacking, it is generally accepted that using low irritating cleansers and humectants (occlusive-containing lipophilic emulsions) improve skin health (18,19). We used a moisturizer with a mix of wax and oils that deliver skin protection and physiologic lipids that are able to traverse the stratum corneum and mingle with epidermal lipids (9). The glycerin, a humectant in the moisturizer, delivers moisture to the skin to promote lipid biosynthesis and NMF formation in dry skin leading to alleviation of xerosis.

Commonly used topical acne therapies, such as benzoyl peroxide, retinoids, antibiotics and salicylic acid usually cause skin dryness and irritation, which leads to increased transepidermal water loss (TEWL) and inflammation (Chularojanamontri et al. 2014). Consequently, many dermatologists are prescribing moisturizers as an additional treatment of acne (Lynde 2008). In fact, moisturizing creams are the most prescribed products in dermatology, essential in maintaining healthy skin as well as in the topical treatment of many diseases (Loden 2003). It was shown that APG-based emulsions as topical drug carriers exhibit excellent skin hydration ability (Tasic-Kostov et al. 2011; Jaksic et al. 2012). Therefore, we also aimed to examine whether APG-based emulsions with spironolactone would moisturize the skin, which could present the additional value of this kind of topical acne therapy.

Coconut oil was selected as it acts as an effective moisturizer on all types of skin, including dry skin. The benefits of coconut oil on the skin are comparable to those of mineral oil. Fortunately, unlike mineral oil, there is no chance of having any adverse side effects on the skin upon applying coconut oil. Coconut oil is, therefore, a safe solution for preventing dryness and flaking of skin. It also delays the appearance of wrinkles and sagging of skin, which normally accompany aging. Coconut oil also helps in treating various skin problems including psoriasis, dermatitis, eczema, and other skin infections. For that exact reason, coconut oil forms the base ingredient of various body care products, such as soaps, lotions, and creams, which are used for skin care. Coconut oil also helps in preventing premature aging and degenerative diseases due to its well-known antioxidant properties (17).
