Inferior heel pain

Plantar fasciitis is the most common diagnosis attributed to inferior heel pain, but there are myriad other conditions that can present with similar symptoms. It is important to take a detailed history and perform a thorough examination prior to attributing a cause, and in cases where there are atypical symptoms further investigation is warranted. A further description of signs and symptoms specific to each differential diagnosis are listed in the appropriate sections; however, it is important to ascertain level of activity, changes in the nature of the pain over time, duration of pain and when it is worse, any aggravating and relieving factors, and the exact location of the pain (Figure 10.1). Indeed, the acronym SOCRATES, commonly taught in UK medical schools, is applicable when assessing inferior heel pain (Table 10.1). Clinical examination should look for scars, foot posture and alignment, Achilles/gastrocnemius tightness and site of tenderness.

Heel pain – plantar fasciitis is inflammatory thickening of the plantar fascia, usually at the origin from the calcaneum and is a common cause of heel pain. The diagnosis is usually clinical but in refractory cases ultrasound can be used to confirm the diagnosis, exclude other conditions such as plantar fibroma or fibromatosis or to guide steroid injection. Achilles tendon pathology is another common cause of heel pain located posteriorly. Chronic tendinopathy is very common as the Achilles tendon also carries a major load during ambulation and is subject to acute inflammation or tears sometimes related to relatively trivial trauma. Ultrasound and MRI can establish the diagnosis of tendinopathy and assess the presence of tears. Partial or complete rupture can be assessed. Dynamic ultrasound can also assess the tendon gap in complete rupture.

Our FE prediction showed that the gastrocnemius-soleus recession (resembled by the reduction of muscle force) reduced the stress of the plantar fascia and supported our hypothesis. While the first ray of the plantar fascia sustained the largest stress among the five fasciae, the stress of the first ray fascia was reduced by 77.8% when the triceps surae load was halved at midstance; and was reduced by 16.9% when the triceps surae load was reduced by 22.2% at push-off. The reduced plantar fascia stress could implicate reduced risk of plantar fasciitis, in addition to the midfoot-forefoot overload syndrome and other foot pathologies.

Plantar fasciitis is a multifaceted foot disorder which usually characterized through heel pain and tenderness on the medial plantar aspect of calcareous tubercle, decreased ankle dorsiflexion [1–3]. It is the most common painful foot syndrome, which could affect the daily activities of the adult population and athletes [4]. Plantar fasciitis may account for up to 15% of all foot symptoms among adults and it impacts women more frequently than men [5,6]. The incidence peaks between the ages of 40 and 60 years [7].
