Drugs may be ingested or smoked with almost no tell-tale signs. Nasal insufflation (snorting) of drugs usually produces nasal congestion and erythema. There may be frequent blowing of the nose, with loss of nasal hair. Sinusitis may be present. Smoking of drugs, though usually no more than a few times daily, produces the same changes in the lungs as are produced by smoking cigarettes. Many abusers also use cigarettes. Intravenous drug use produces many physical findings (Table 15). The veins may be scarred, thrombosed, or pigmented from chronic injections.

The first line of respiratory tract defense against infection is the anatomic configuration of the upper respiratory tract. Multiple bifurcations and nasal hair result in removal of large suspended particles from the air. Coughing and sneezing reflexes as well as the mucociliary transport system are helpful in clearing particles from the terminal bronchioles to the larynx.19

The nostrils are lined with skin and the nasal hairs project from it into the airway. Through the nasal valve there is squamous epithelium undergoing a gradual transition just beyond the anterior ends of the turbinates to ciliated secretory respiratory epithelium.17 Goblet cells are interspersed with ciliated cells throughout the main passage.18 The submucosa in the main passage contains a rich vasculature and a myriad of submucous seromucinous glands. Through the main passage, the surface is covered with airway secretions reaching at least some 10 to 12 jim above the cell surfaces. In the nasopharynx there is a transition again to squamous epithelium, although ciliated cells are found especially over the adenoids which project from the posterior nasopharyngeal wall.

The vestibular area, the front part of the nasal cavity within the nostrils, is approximately 0.6 cm2. The nasal hairs, also called vibrissae, filter the inhaled particles which serve as a barrier system where its surface is covered by stratified squamous keratinized epithelial cells (Illum 2003; Gallo et al. 2016). The atrium is a segment between the nasal vestibule and the respiratory region. The anterior segment of the atrium consists of stratified squamous epithelium and the posterior segment consists of a pseudostratified columnar cells adorned with microvilli (Jadhav et al. 2014).
