Other viral infections
Published in Hung N. Winn, Frank A. Chervenak, Roberto Romero, Clinical Maternal-Fetal Medicine Online, 2021
The rash associated with human parvovirus B19 infection occurs 17 to 21 days after infection and is probably secondary to antigen-antibody reaction (35). Arthritis or arthropathy, a probable postinfection phenomenon, may manifest with a purpuric rash—like illness, often associated with other signs of vasculitis. This rash may begin as a vesiculopapular eruption and is often distributed in the buttocks and lower extremities.

The shape of the buttocks is an important aesthetic feature in many cultures. As with most other areas of the body, the buttocks are subjected to age-related changes. Gluteal ptosis, as a result of volume loss and skin laxity, and alterations in the skin morphology, such as striae distensae and cellulite, has led to an increased patient demand to rejuvenate and/or reshape the buttocks. A wide variety of techniques have been introduced to rejuvenate and reshape the buttocks. Proper evaluation of the patient's wishes is essential in the decision of which treatment will lead to the most optimal results. CaHA can be used as a monotherapy, or as part of a multidimensional approach, in the treatment of skin laxity and cellulite. Lifting the buttocks (reshaping) may also be achieved with CaHA, although these results are less pronounced than for example a surgical buttock lift.

In addition, the patient’s skin lesions on the lower limbs and buttock also shown significant improvement after treatment. Before hAMSC administration, the patient had widespread irregular ulcers on the buttocks and lower extremities, with scabs and purulent odor secretions on some wounds. Additionally, brown pigmentation was visible at the edge of the ulcers. After 4 months of treatment, the buttock ulcer had notably reduced in size and become shallow, with some regenerative tissues visible. Pain had considerably reduced, with the infection under control, and no odor present. After 12 months, most of the buttocks’ wounds had healed, except for three small ulcers and a few erosive surfaces. Pain was almost non-existent. After 15 months, the skin scar on the buttock had healed and became painless [11,20]. Hence, the patient got great relief of wound pain and the scores of pain VAS decreased from 10 to 0 after 15 months hAMSC treatment. Likewise, the score of BWAT decreased from 65 to 13 points, accompanying with substantial improvement in quality of life after hAMSC application for 15 months (Figure 1(H,I)). Either inflammation status or immune disorder was ameliorated after hAMSC administration for one year, with tumor markers in normal ranges [11].

A 43-year-old diabetic man presented with a 14-years history of HS which was unresponsive to systemic antibiotics, isotretinoin, and adalimumab treatments. Due to Hurley stage III and refractory disease, adalimumab treatment was switched to secukinumab therapy. However, 10 days after the sixth dosage of secukinumab therapy, he noticed hyperemia and edema evolving to painful ulcerations on the axillary folds where diffuse sinus tracts and abscesses compatible with HS lesions were located (Figure 1(a)). Additionally, there were inflammatory pustules and papules on his sternum, back, legs, and buttocks (Figure 1(b)). Shortly after, some of these lesions on the legs and buttocks became ulcerated. During hospitalization, he developed a single genital ulcer and two oral ulcers that resolved spontaneously within a week. No recurrent or new ulcers developed thereafter (Figure 1(c–d)).

After sixty-two patients were excluded as they had missing data, a total of 419 patients were included. 240 (57.2%) of the patients were male, and the median age was nine years (IQR 5–14). When patients were classified into age groups using a standard classification [14], most patients were seen in the 12–18 age group (40.3%). Regarding the seasonal distribution, most patients were seen during summer (Table 2). 51.5% of the patients (n = 216) had contacted dogs, and 47.4% had contacted cats (n = 199). In most cases, the contacted animal was a stray animal (74.7%). Most of the contacted animals were either not vaccinated, or their vaccination status was unknown (93.3%). Most cases were located at one site (91.6%). As expected, extremity injuries were observed the most (80.9%). Hand injuries were the most common among extremity injuries. Head and neck injuries were the second most common (17.6%). Twenty-two patients presented with a genital or buttock injury.
