Buttock augmentation is a surgical procedure for reshaping and increasing the size of the buttocks, either to create a balanced look, or to simply enlarge the butt area. This is accomplished with either an implant (silicone) or through micro fat grafting. This procedure has become more in demand in recent years due to people wanting to look like celebrities, e.g., Jennifer Lopez.

At present, the anatomical mechanism of FINTPE caused by genital injection is not precise. In general, the injury of blood vessels is regarded as the cause (14). For vaginal injection, with abundant vessels and narrow local space in the vagina, it is likely to produce excessive pressure, severe vascular injury, and filler displacement during injection (14,17,20,31). It is supposed that the fillers may be inadvertently injected into the blood vessels through the vaginal wall and drained to the internal iliac vein through the venous plexus on both sides of the vagina and through the uterine vein, bladder venous plexus, and rectal venous plexus. The fillers in the internal iliac vein then enter the inferior vena cava reflux system and reach the right atrium. Finally, the fillers pass through the right atrium and right ventricle and successfully cause embolism by pumping into the pulmonary arteries (15). This process is kind of like the mechanism of the pulmonary embolism caused by buttock augmentation with fat grafting (34,35). We summarized that the mechanism of FINTPE has two elements: 1. There are many blood vessels in anatomical parts, especially thin-walled veins with large blood flow, which largely increases the possibility of vascular damage. 2. Large injection volume within a small injection space leads to excessive local pressure, pushing the emboli to enter the blood vessel.

Some feminine trans individuals augment their feminine appearance through the use of exogenous hormones, fillers (e.g., injectable silicone), and surgical procedures (Boellstorff, 2004; Grant et al., 2011; Howe et al., 2008; James et al., 2016; Kulick, 1997; Meyerowitz, 2002; Milan, 2017; Prieur, 1994; Puckett et al., 2018; Scheim & Bauer, 2015; Stief, 2017). Feminizing surgeries can include breast, hip, and buttocks augmentation, facial plastic surgery, and genital reconstruction. A significant number of feminine trans individuals do not undergo any of these procedures but augment their feminine appearance in nonpermanent ways (e.g., makeup). Others modify their bodies using hormones, but not surgery (e.g., Grant et al., 2011; James et al., 2016; Kulick, 1997; Prieur, 1994; Scheim & Bauer, 2015; Stief, 2017). Feminine trans individuals who undergo vaginoplasties appear to be in the minority (e.g., James et al., 2016; Kailas et al., 2017; Kulick, 1997; Prieur, 1994; Puckett et al., 2018; Scheim & Bauer, 2015; Stief, 2017; Ten Brummelhuis, 1999).2For some feminine trans individuals, the cost of genital reconstructive surgery may be prohibitive; others may feel that the surgical outcomes are sub-optimal; still others may elect not to undergo these procedures because they do not feel that having a penis is inconsistent with a feminine gender identity.

In buttocks grade 3, there is a combination of severe ptosis and flaccidity of the buttocks. This may call for an extended procedure with for example posterior lift of the lower back combined with infragluteal fold plasty and/or reconstructive autologous buttock augmentation for projection.
