A moderate, uni- or bilateral pleural exudate develops in up to a third of patients with the features of parenchymal APT.4 A lone free-flowing effusion is unusual. Pericardial effusion can be present as well. One patient on high-dose amiodarone (1.6 g, followed by 1.2 g daily) developed bilateral exudates having the same biochemical characteristics; the effusions resolved on drug discontinuation.134 In another patient, a large bilateral serosanguinous effusion predated the onset of classic APT.135 The pleural fluid in amiodarone-associated effusions is a lymphocyte-rich136 or lymphocyte- and neutrophil-rich exudate, with a range of protein concentration of 2.85.5 g/dL.134 Foam cells resembling those in the BAL fluid can be present in pleural fluid.135 On pathology in one case, there was pleural thickening and foam cells were present in pleural tissue.134 When pleural effusion develops during treatment with amiodarone, this is often against the background of heart failure which can also manifest with pleural transudate or exudate.137

Pleural effusion refers to accumulation of fluid in the pleural space, between the visceral and parietal pleural layers. The radiographic appearances of pleural effusion are generally the same regardless of the nature of the fluid, which may include transudate, exudate, blood, pus (empyema) or lymph (chylothorax).

GBM 7: The density value of blood plasma 1.025 gm/cm3 was assigned to the slab on presumption that oedematous fluid is a transudate derived from the blood through capillary fluid exchange. RCCT521; characterises resected cavity with cystic fluid and compositional equivalence to GLCT521.The computed slab dimension was R = 2.4232 cm and Z = 1.755 cm.
