Hypothermia following cold-water immersion treatment for exertional heat illness

Patient B was a 28yo otherwise-healthy male who presented to the mile 22 medical tent complaining of feeling “hot” and was found to have a temperature of 41.7°C. He was treated with CWI “twice” per medical tent documentation, implying two distinct immersion baths, before being transferred to our ED where he complained of feeling cold. His temperature was 34.7°C. Laboratory analysis showed mild hypernatraemia (serum sodium of 148 mEq/L). The patient was given warm IV saline and covered in warm blankets. He was discharged with a temperature of 36.3°C.
