Cryptococcosis

Cryptococcosis is a fungal disease caused by Cryptococcus neoformans, which can be transmitted through aerosol spore exposure and is most likely to afflict persons with AIDS. Cryptococcosis, caused by Cryptococcus neoformans, a yeast pathogenic to man and animals, has been isolated from soil in many parts of the world but is particularly associated with excreta of pigeons. In man it usually develops as an infection of the central nervous system. However, it is presumed that this results from hematogenous spread of the organism from primary lung lesions. Very small basidiospores of the sexual (teleomorphic) state of this fungus (viz., Filobasidiella neoformans) are thought to be the means of aerial dispersal. Slimy capsules of the pathogenic yeast phase may be difficult to see in microscopic examinations of body fluids. In culture it is slow-growing and sensitive to cycloheximide. Blood agar is a recommended medium for growth at 37°C. Intracerebrally inoculated mice usually die in 4 days to 2 weeks. Brain material should be examined for the encapsulated, yeast-like cells. Serological diagnostic methods are not very satisfactory.

Fluconazole (F) is an antifungal triazol type used in the treatment of cryptococcosis, hysto-plasmosis [29], and in the prophylaxis of infections where other anti-fungals have failed or have been not tolerated by the patient, including candidiasis, onychomycosis and cryptococcal meningitis [30].
