Compared with Caucasian women, Asian women start injectable treatments earlier, even though their aging symptoms onset later. Neoteny is observed in many mid-aged Asian women without treatments. That explains why a juvenile appearance may embarrass Caucasian females but is not strange for middle-aged Asian women. The attitude toward injectable treatments is open, especially in Eastern Asia. Peer pressure and encouragement reinforce the motivation to keep oneself young and looking good. Aesthetic works in a nonaging subject are simply directed to enhancement rather than antiaging (Figure 11.A.10a and b). The wide structure of the Asian skull pulls and holds the soft tissue but leaves the front of the face flat. Flatness of the face should be enhanced by midface augmentation, including the nose, medial cheek, paranasal, glabella, and the forehead. The midline of the face should be aligned and balanced when injection augmentation is planned. Premaxillary cheek augmentation that corrects the suboptimal cheek fullness and camouflages the problem of prominent cheek, when it is not accompanied by comparable nasal augmentation, further worsens the nose's sinking into the face. Wide cheekbones could be camouflaged by injection treatment on the medial cheek, submalar area, and the temples.

The first step in cheek augmentation is to assess your patient to ensure they are suitable for the treatment. Cheek augmentation may not only restore lost volume in the cheeks but also can decrease the visibility of bags under the eyes or periocular wrinkles on smiling. A quick and easy method of assessing for volume loss in the cheeks is to run your index finger from the zygoma to the nasal bone in a sweeping motion. Corrugation of the skin whilst you perform this motion is likely to indicate volume loss in this area. If a patient is seeking cheek fillers without volume loss, then this test may not be overly useful, and it may not be in their best interests to proceed with treatment.
The middle third of the face

Accordion lines can be observed in the cheeks, lateral to the nasolabial folds and marionette lines. These lines are the direct result of anterior and inferior descent of the cheeks during the ageing process, combined with skin laxity. The ideal treatment for this indication therefore includes a lifting technique to reposition the cheeks to their original anatomic location. Lifting of the cheeks is discussed in more detail in the chapter on cheek augmentation. In this chapter, we focus exclusively on the wrinkles themselves.

Cheek augmentation is a modern cosmetic procedure to fill out sunken cheeks and change the contour of the face. Non-surgical treatments, such as autologous fat graft and injectable dermal fillers, can give a more youthful appearance by restoring cheek volume. Injectable dermal fillers have immediate advantages, requiring minimal downtime, and allowing individuals to return to work and their normal daily activities almost immediately post-treatment (5).
