HA soft tissue fillers consist of long chains of hyaluronic acid. Most dermal filler products will consist of HA cross-linked with a chemical such as 1,4-butanedioldiglycidyl ether (BDDE) for Restylane®, Belotero®, Teosyal®, Hyabell®, Stylage® and Juvéderm® and suspended in a physiological or phosphate-buffered solution [8].

There are few to no studies that specifically examine the use of filler agents in skin of color. The safety and use of Restylane (Galderma Laboratories), a hyaluronic acid, have been studied off-label in patients with Fitzpatrick skin types IV to VI in isolated cases. There were no transient or permanent adverse outcomes among the type IV to VI subjects in those cases. If proper injection techniques are used, patients with Fitzpatrick skin types IV to VI can benefit from this kind of treatment [55].

Hyaluronic acid-based fillers are ideal because they are less immunogenic and have a longer duration effect than collagen. Patient tolerability is good. Hyaluronic acid as smaller particles is marketed by Q-Med Uppsala as Restylane and larger particles as Perlane. Restylane usually lasts approximately 6 months and Perlane can last 6–12 months. The same company also produces hyaluronic acid of even greater size (Macrolane). The hyaluronic acid injections can be repeated at 4–6 month intervals. These fillers are a useful adjunct in facial cosmetic procedures. They very rarely cause allergic reaction. The US Food and Drug Administration (FDA) has approved hyaluronic acid (Restylane) as a filler for nasolabial fold augmentation. Injection-related side effects include bruising, erythema, pruritus and discolouration. Sometimes granulomas occur and they respond well to injection of corticosteroids followed by massage.

1,4-butanediol diglycidyl ether (BDDE, the first linker from the bisepoxide family) was the first crosslinker to be used in a commercially available HA-based filler. This filler, Restylane, was introduced in Europe in 1996 and FDA-approved in 2003 and is still sold worldwide [24–26]. Divinylsulfone (DVS)-crosslinked HA fillers Hylaform (HA from rooster comb) and Captique were introduced on the US market in 2004, while a p-phenylene biscarbodiimide (BCDI)-crosslinked HA filler Hydrelle (formerly named Elevess) was FDA-approved in 2006; none of these remain on the market. Diepoxyoctane (DEO, the second linker in the bisepoxide family)-crosslinked HA fillers were introduced in Europe in 2005 under the name Puragen [27] but are also no longer commercialized. Finally, polyethyleneglycol diglycidyl ether (PEGDE, the third linker from the bisepoxide family)-crosslinked HA fillers were recently introduced in Europe in 2012 within the Neauvia product line.

The HA injection was performed in the office setting under topical anesthesia using RestylaneR (Q-med, Sweden) in an off-label manner. After Proparacaine (AlcaineR, Alcon, USA) instillation, the upper eyelid was everted and HAG was injected transconjunctivally into the levator plane using a 30-gauze needle (Figure 1). One to three injection sites were chosen to properly recreate the normal curvature of the upper eyelid. After injection, the patient was monitored for 10 minutes, to ensure no intravascular complications occurred. No post-injection topical or oral antibiotics were administered.

Volume loss and muscular hyperactivity hugely contribute to the formation of wrinkles in the aging process and the development of reversible minimal invasive techniques for wrinkles and volume correction has tremendously increased in the past few years (9,10). Among others, injecting fillers, especially the use of hyaluronic acid to correct wrinkles have become a standard therapeutic method in modern cosmetic practice. Restylane is biphasic injectable filler based on individual particles. It was firstly introduced to the Europe market in 1996 for the correction of moderate facial wrinkles, facial folds, and lips (6,11–15).
