These are not common injuries. Fortunately, persistent instability is infrequent. Complications vary and seem to be related to the degree of energy of the injury. Post-traumatic arthritis evident on radiographs is common although the symptomatic effects of these changes may vary. These may be a result of injury to the joint surface, occult fractures or persistent instability. Open fracture dislocations with associated injuries to the nerves, vessels and related tendons (tibial nerve, posterior tibial artery or posterior tibial tendon) have been associated with worse outcomes.

Good outcomes can be expected and even some residual deformity does not seem to produce much disability. Complications include nonunions and malunions, which are symptomatic if they produce abnormal plantar pressure or irritability with shoewear. Joint stiffness and post-traumatic arthritis, from articular fractures, may also develop.

When we analysed the pathology that caused the surgical indication, we found that the results of the TWF were worse in patients with rheumatoid arthritis than in patients with post-traumatic arthritis [21] due to poor bone quality, the higher risk of complications and the increase in mechanical stress caused by the fusion on the other upper limb joints, which are usually also compromised by the disease. This, combined with the rheumatic patient’s low physical demand made them the ideal candidate for TWP at the time [22–24]. However, the young post-traumatic patient with no underlying pathology has good bone quality, and their work prospects after surgery will determine which technique is more suitable. With fourth-generation designs, TWP indications for post-traumatic patients are more and more common [10,25].

Another illustrative example of a slight TM misstep but in the field of orthopedic surgery was the widespread adoption of metal on metal (MoM) bearing surfaces in total hips6. MoM was not an entirely new concept in the mid-to-late 1990s when MoM became mainstream. Adopters of the MoM hip saw two distinct advantages. Typically, a joint replacement procedure is reserved for an elderly patient with advanced joint degeneration who had failed non-operative modalities. Simultaneously orthopedic surgeons were seeing younger and younger patients in need of a total hip replacement. Often their hip joints were deteriorated as the result of post-traumatic arthritis, avascular necrosis, or another disease process. These younger patients would benefit most from a hip implant that would survive their lifetime and not wear out to the point of needing a revision total hip surgery which are very complex and fraught with complications such as infection implant subsidence and so forth, and these younger patients are fairly active so they require additional stability from the replaced joint so that upon normal activities of daily living – including sitting, bending or squatting down – the joint would not dislocate.

Diseases of the TMJ span a broad spectrum and can be classified as low or high inflammatory diseases (14). Low inflammatory disorders such as post-traumatic arthritis or degenerative causes are unlikely, mainly owing to their rather proliferative character and also because the overall dental status shows little pathology. Furthermore, post-traumatic arthritis tends to be unilateral.
