Patients with glycogen storage disease type Ib [4] have the same type I clinical phenotype but, in addition, they have neutropenia and impaired neutrophil function (see Table 58.1) [58]. As a consequence, they have recurrent bacterial infections, inflammatory bowel disease, and ulceration of oral and intestinal mucosa [59–63]. Fecal α-1 antitrypsin is increased. Biopsy of the colon reveals inflammation [23]. Among 288 patients with type I glycogenosis, 57 had type Ib [62]. Neutropenia was found in 54. It is often documented in the first year of life, but may be noted first between six and nine years. It may be persistent but, more commonly, it is intermittent. Among 18 patients with neutropenia in whom neutrophil function was studied, it was abnormal in all. Apoptotic neutrophils were documented by increased activity of caspase, condensed nuclei, and perinuclear clusters of mitochondria in this disease, but not in other neutropenic disorders, such as Shwachman Diamond syndrome [64]. Perioral infections occurred in 37 patients, perianal infections in 27, and protracted diarrhea in 23. Inflammatory bowel disease was documented by colonoscopy or roentgenographic examination. Inflammatory bowel disease was not observed in the absence of neutropenia. Two Japanese patients were reported with no evidence of neutropenia and no recurrent bacterial infections [63]; otherwise, symptoms were typical of type Ib glycogenosis and there were mutations on both alleles of the gene.

White blood cells may also increase or decrease in number. Neutropenia is a reduction in the number of neutrophils in the blood. It occurs in a wide variety of diseases, including certain hereditary defects, aplastic anemia, bone marrow tumors, and acute leukemias. Granulocytopenia is a more general term describing a reduction in the number of granulocytes (leukocytes), including neutrophils. Agranulocytosis applies to diminished or absent granulocyte production, essentially the same disorder. Whichever term applies, granulocytopenic conditions all result in increased susceptibility to bacterial infection and mucous membrane ulcerations,: usually in relation to the degree of the deficiency.

All patients underwent complete blood count (CBC) and biochemical examination (including Ca, ALB, β2-MG, Crea, HDL-C) when they were diagnosed with MM. ISS was used as staging criteria. The CBC was detected using a haematology analyzer XE-2100(Sysmex Corporation, Kobe, Japan), and the neutrophil count to lymphocyte count ratio (NLR) was calculated. The biochemical indexes of ALB, HDL-C, Ca, β2-MG and Crea were detected by an automatic biochemical analyzer Roche c702(Roche Diagnostics GmbH, Mannheim, Germany). The ratio of NLR to ALB (NAR thinsp;= NLR/ALB*100) and the ratio of NLR to HDL-C (NHR = NLR/HDL-C*100) were calculated. Overall survival (OS) was defined as the time from diagnosis to death for any cause. Disease-free survival (DFS) was defined as the number of days from diagnosis until local or regional recurrence or distant metastasis.
Impact of socio-behavioral measures implemented during the SARS-CoV-2 pandemic on the outcomes of febrile neutropenia episodes in pediatric cancer patients: a single center quasi-experimental pre-post study

Patients analyzed were admitted with a diagnosis of febrile neutropenia, defined as an absolute neutrophil count (ANC) less than 0.5 K/µL or 1 K/µL with anticipation to decrease, a fever of 38 °C for greater than 1-h or single fever greater than 38.3 °C.24,25 All FN episodes included in analysis received inpatient management; location of first fever is reported as outpatient or inpatient setting. Oral temperature measurements were preferred and obtained by the primary caregiver if at home or a healthcare provider during admission using a digital thermometer as documented in the electronic health record. Admissions were excluded from the data analysis if they did not meet FN definition or bone marrow transplant recipients to restrict the analytic group to the general oncology cohort. Patients were allowed to have multiple FN episodes provided an interval of greater than 7 days was observed between febrile events. There were 9 FN episodes (1.8%) that occurred inpatient during an admission of a previous FN episode.

Neutrophil count decreased was the most frequent TEAE in this study, with comparable incidence between groups. This frequency is aligned with low neutrophil counts being reported as marked laboratory abnormalities in studies of reference tocilizumab [26,27]. Low neutrophil counts are also a consideration for dosing included in the EU-tocilizumab prescribing information [5]. In the current study, no signs or symptoms of infection were reported in participants experiencing decreased neutrophil counts. The incidence of TEAESIs of hepatic event, ISR, and hypersensitivity was relatively low, and was comparable between groups. The incidence of TEAESIs of ISR was generally similar to that observed in a phase I study which compared another candidate tocilizumab biosimilar with the reference product (4.8% overall) [15].
