Vardenafil (Levitra) is available in 2.5, 5, 10, and 20 mg tablets. The typical starting dose is 10 mg, which can be increased to 20 mg if patients do not achieve an adequate response. Lower doses are appropriate for those who have side effects with 10 mg and for those patients on antifungal or antiretroviral therapy. Vardenafil reaches a maximum concentration in about 45 minutes and has a half-life of about four to five hours. Patients should be advised to take the agent about 60 minutes before intercourse. Side effects are similar to those seen with sildenafil. Headache (15%), flushing (11%), rhinitis (9%), and dyspepsia (4%) are the most common. Since the agent is much more selective for PDE-5 than for PDE-6, the effect on blue–green color discrimination observed with sildenafil is not observed with vardenafil (22).

PDE5 inhibition by tadalafil decreased both in vivo and in vitro airway reactivity in lung tissue and blood eosinophils in ovalbumin-sensitized guinea pigs [25]. Sildenafil, is a short-acting PDE5 inhibitor already used in erectile dysfunction and pulmonary hypertension. It presents bronchodilator effects because of relaxation of smooth muscle cell by increasing levels of cGMP mainly through nitric oxide (NO) in large airways and for that reason, it has moderate effects in asthmatic patients [61]. Sildenafil does not affect the early or late asthmatic response, but it inhibits AHR by the NO-cGMP pathway or the accumulation of inflammatory cells [62]. Vardenafil, is already used for erectile dysfunction [63]. Vardenafil inhibits parasympathetic function of the trachea as well as metacholine and electrical field stimulation-induced contraction of rats’ trachea [64]. PDE5 inhibitors’ side effects include headache, flushing, nasal congestion, dyspepsia and myalgia, due to the vasodilatory effects on the capillary smooth muscle in other parts of the body [64,65].

Vardenafil is a highly selective and potent PDE5-I. Despite being similar in chemical structure, it is more potent and selective than sildenafil. Vardenafil has a faster onset of action compared to other PDE5-Is. Indeed, 53% of patients can obtain an erection sufficient for penetration at just 25 min. As well as sildenafil, vardenafil absorption is impaired if the drug is taken after a fat meal. Dose-dependent side effects include headache (21%), flushing (13%), and dyspepsia (6%) [29]. A recent study reported that vardenafil successfully prevented the development of DM-associated heart failure with preserved ejection fraction (HFpEF). Thus, the use of PDE5-Is as a preventive approach might be a promising option for the management of HFpEF patients with DM [30].

Vardenafil, a PDE-5 inhibitor, is used to treat erectile dysfunction with its vasodilatory effect due to increased cGMP and nitrous oxide levels [37]. The potential for these compounds to be used in the central nervous system (CNS) is dependent on their ability to pass across the blood-brain barrier (BBB). Sildenafil has been reported as clearly crossing the BBB (FDA 1998), and vardenafil has also been shown to do so [38, 39]. Anti-inflammatory and anti-oxidant effects have been reported for vardenafil [40]. Moreover, vardenafil and sildenafil have enhanced memory function, cognition and spatial learning in rat studies [41–43]. However, there are no studies on the effects of vardenafil on autism, especially in rats with an ASD-like phenotype.
