Isoetharine, an oral sympathomimetic drug used orally or intranasally (1-percent solution) to treat bronchospasms. Isoetharine is the most selective beta agent of this class. It has weak bronchodilation properties, but does not stimulate the heart as other beta agents do. No published studies are available on congenital anomalies in infants of mothers exposed to isoetharine during pregnancy. No animal teratology studies in animals have been published. Isoproterenol is a closely related drug. Under the old system, it is a FDA category C.

Many other compounds satisfying the requirements for good broncho-dilatation with less side effects and better pharmacokinetic properties were studied. One of these was the compound soterenol (MJ 1992), in which the 3-hydroxyl group on the catechol nucleus of isoprenaline was substituted with a methane sulfonamide.45 But only four beta agonist bronchodilator aerosols were available for use in asthma in the United States at the beginning of the 1970s, i.e., adrenaline, isoprenaline, isoetharine, and orciprenaline (Figure l).46

Albuterol (Salbutamol) is created by modifying the primary catechol nucleus common to the naturally occurring adrenergic neurotransmitters adrenaline and noradrenaline. Albuterol is longer lasting than isoprenaline and isoetharine because it is not broken down by COMT [30]. The albuterol molecule (Figure 3) can directly reach the β2-receptor from the extracellular compartment because of its hydrophilic properties. Because of this, albuterol can effectively bronchodilate in as little as 2–3 minutes and reach maximum bronchodilation within 15 minutes after inhalation. It binds to the AR weakly, which allows it to diffuse into the systemic circulation only after a short (3–6 hours) duration of action [36]. Given these characteristics, albuterol is considered the preferred go-to relief medication for asthma management [9].
