Turning to the more specific topic of his lecture, Falloppia discussed the causes of ulcers, in general. Ulcers developed when evil, corrosive humors or fluids (“juices”) flowed toward the skin. These “juices” resulted from a cacochymia of the body or one of the major organs. There also were excrements from individual parts of the body, however, he added, and fluids that originated in the affected part itself, as in the case of frostbites. Certain ulcers were ultimately due to contagion, which was transmitted by vapors or direct contact. Thus, lung ulcers resulted from foul vapors, which were “infected” with putrefaction. Just sharing the bed with someone who suffered from scabies could lead to scabies. Sexual intercourse with a woman who had a genital ulcer would cause an ulcer also in the male member.30

After examining the patient to confirm the presence of a genital ulcer, the treatment appropriate to local aetiologies and antibiotic sensitivity patterns should be given. Ulcers caused by herpes simplex virus (HSV) in HIV-positive patients can be atypical and persist for longer than usual, so HSV infection should be strongly considered as a differential diagnosis in areas where HIV infection is prevalent.

This venereal infection is caused by the Gram-negative bacillus Haemophilus ducreyi. One to 5 days post-infection, a soft sloughy ulcer appears on the areas of contact on the penis or vulva. Other sites may be affected, and inguinal adenitis occurs in 50% of patients, usually unilateral in distribution. Both the genital ulcer and lymphadenopathy are painful, a characteristic feature of this condition.

In the present study, the results revealed that the most common finding was a recurrent oral ulcer and the second most common finding was a genital ulcer. While some studies have shown that genital ulcers were less common in PBD,2,18 opposite results have been reported as in agreement to our findings.3,16 Our results have shown that the rate of ocular involvement is 43.4%. In previous studies, the incidence of ocular involvement has been reported in a range of 8.7% to 80.0%. Moreover, in accordance with our findings, it has been seen more frequently in boys.2,3,8,16,17 In addition, ocular involvement was observed to be less common in pediatric patients compared to the adults.18,19 Similar to previously published studies, our results showed that a high prevalence of family history was associated in pediatric patients when compared to the adults and this strongly suggests the involvement of genetic background.8,16 Our findings suggest a less neurological and vascular involvement when compared with other published studies.8,16,17 In the present study, comparison of PBD patients with and without ocular involvement did not reveal any significant difference in terms of sex, age at disease onset, follow-up period, pathergy test, skin lesions, arthritis, and arthralgia. Family history was associated strongly in patients with ocular involvement. Likewise, genital ulcers were higher in patients without ocular involvement.
Latent class analysis of symptoms for sexually transmitted infections among Iranian women: Results from a population-based survey

Even though the response frequency of symptoms varied in different age-groups, the differentiation pattern between Class IV, with the least prevalence, and Class I, with the highest prevalence, remains unchanged. The possibility of response to the genital ulcer is lower in the 18–25 age-group (Figure 2(B)). The genital ulcer seemed a weak symptom for participants’ classification. Members of Class IV had the highest response to the frequent urination symptom. Differentiation of Classes I and II are not feasible using this symptom. In the 26–35 age-group (Figure 2(C)), Class IV members have been completely distinguished from other classes. People in Class IV within this age range had the highest response to LAP, AVD, and dyspareunia. The possibility of response to genital ulcer was greater in the 26–35 age-group than other groups. The frequent urination was a weak symptom for distinguishing Classes I and III or II and IV. Although Class IV maximum rate of response was similar in both age groups (26–35 and ≥36), Figure 2(D), genital ulcer reports were lower in the 36 or older participants. In the ≥36 age-group, genital ulcer, AVD, and LAP were poor indicators for distinguishing Classes II and III. Classes I and II could not be distinguished based on post-coital pain.

We also investigated drug administration (especially colchicine) during pregnancy. Colchicine (1–2 mg/day) was used in six pregnancies (9.1%) who were already in the active stage of BD. The altered symptoms were found to be oral ulcer in three, genital ulcer in two and uveitis in one among these patients. There was no colchicine treatment in the remaining pregnancies. One patient who was not at the active stage of BD but having rheumatoid arthritis was under the treatment of low dose corticosteroid (methylprednisolone 1 × 4 mg/day). This pregnancy was ended up with an early spontaneous abortion.
