Adenomyomatosis is found more commonly in females and has a strong association with gallstones. The classical appearance on ultrasound is of cholesterol-filled Rokitansky-Aschoff sinuses which appear as hyperechoic foci in a thickened gallbladder wall. On MRI the ‘string of beads sign’ refers to the characteristic curvilinear multiple rounded T2 hyperintense intraluminal cavities. Gallbladder carcinoma is the main differential.

The size and shape of the fasting gallbladder can be assessed, including the presence of folds and septations. Contraction of the gallbladder following a fatty meal may give improved views of the cystic duct and may better demonstrate Rokitansky-Aschoff sinuses.

Microscopic examination of the cauliflower mass revealed a tubulopapillary growth composed of complex branching papillae with fibrovascular cores. Some of the papillae were lined by stratified pancreaticobiliary epithelium, others by intestinal type epithelium with goblet cells and still few others were lined by foveolar gastric type epithelium. No dysplasia was noted. Most of the papillae were edematous and showed numerous histiocytes in their cores. Neither invasion in the cores nor in the underlying gallbladder wall was detected. The growth was seen reaching the cystic duct resection margin. The wall of the gallbladder showed Rokitansky–Aschoff sinuses, blotches of calcification, acellular mucin pools and scattered few papillary projections with similar histologic features away from the main mass (Figure 4).

There are a few benign gallbladder morphologies which may present macroscopic abnormalities. Some gallbladders present with Rokitansky-Aschoff sinuses also known as adenomyomatosis (Figure 3(a)), which is a benign condition and is characterised by pseudodiverticula of the gallbladder reaching into lamina muscularis as a result of hyperplasia and herniation into the gallbladder lumen [13]. Distinguishing between these sinuses and a possible (pre-)malignancy can be challenging.
