MRI is considered by many to be the gold standard test for MSK problems due to the excellent soft tissue resolution achievable and ability to image in multiple planes. However, ultrasound is a quicker and cheaper alternative for accessible anatomical areas and may be the first-choice imaging modality. MRI can assess subtle soft tissue injury by detecting signal change in muscle and bone that other techniques cannot achieve. Sensitive depiction of tissue planes allows mass lesions and distortions to be detected accurately, and the use of contrast can further improve the local staging of tumours. It is the optimal investigation for suspected bone, joint and soft tissue malignancy, though CT may be needed for additional information.

Injection acupotomy therapy is a minimally invasive treatment method of traditional Chinese medicine that integrates needle-knife loosening and local injection. It was invented by Professor Hanqing Wu, a famous Chinese researcher who is an expert on minimally invasive procedures. The procedure is based on the nine-acupuncture of ancient China [27]. Injection acupotomy was given the Chinese utility model patent certificate in 2006 (patent number: ZL 2005 2 0031398.1). The equipment has a hollow pipe in the center of the acupotomy, through which the fluid can be injected into the soft tissue, the acupotomy can be released at the same time as the drug is injected (Figure 7). This procedure improves anti-inflammatory and analgesic effects, reduces postoperative recurrence, and reduces the pain of patients. It has a clear curative effect on pain and dysfunction caused by soft tissue injury.

Penile soft tissue injury is another common means of injury. These injuries typically occur by means of strangulation or entrapment. Strangulation is a form of compartment syndrome frequently occurring in adults for means of prolonging erections for sexual gratification. Urgent treatment is required to avoid permanent damage [14]. The treatment of choice often depends on a multitude of factors such as material of the strangulating object and availability of resources. Commonly used techniques are use of cutting devices, string and aspiration method, and degloving operation. The first step in treatment typically starts with application of a lubricant with an attempt of manual removal. This process is usually performed concurrently with the string and aspiration method, where blood is aspirated from the corpora with an 18-G needle to achieve decompression followed by tightly winding string distal to the ringed object in hopes of sliding the ring over the string. If this fails, more drastic measures are taken by means of intraoperative bone or wire cutters and, possibly, power drilling machinery. After object removal, urethral inspection and possible skin grafting may be required depending on the extent of injury [14,15].

Wound healing is a normal biological process following a soft tissue injury and it’s a highly organized cascade of events that includes phases such as haemostasis, inflammation, proliferation and remodelling [1]. Therefore, interference with any of the four stages during the wound healing can result into an impaired healing process [2]. During chronic wounds, regeneration of tissue is arrested in the inflammatory stage that can result to pathologic inflammation and failure of initiation of advanced stages of wound healing [3]. Mulder et al. stated that there is an abnormal production of inflammatory cytokines when there is a prolonged inflammatory phase [4]. Hence, the increased presence of neutrophils cells can act as a marker for chronic wounds [5,6]. According to Guo et al. the extracellular matrix (ECM) has been degraded as a result of an increase in inflammatory cells and which ultimately leads to increase in secretion of matrix metalloproteinases (MMPs) and loss of vital growth factors necessary for proper wound healing [7,8]. Table 1 shows the important factors involved in chronic wounds.
