Hygiene

It is important that a person who is nearing the end of their life is kept as comfortable as possible. Mouth care should be given frequently but as invasively as possible. Teeth can be gently brushed twice a day together with the frequent use of dry mouth gels or the application of moisture by using a soft, small-headed toothbrush dipped in water. Dentures will still need to be cleaned daily if worn, however the patient may not choose to continue wearing them. Pain-relief gels can be prescribed, as can pain-relief mouthwashes and sprays. Suction may be required (Doshi, 2016).

Electrically powered toothbrushes have become popular in recent years and early studies indicated that they cleaned more efficiently than manual brushes. However, longer-term studies found no difference.8,51,98 If should be noted that electric toothbrushes are of significant value for handicapped persons, and where nurses or others have to clean the teeth of people such as the institutionalized elderly. More frequent brushing tends to give better plaque control and less gingivitis, but it has been shown that the efficiency of toothbrushing is more critical than frequency.52,95 Careful brushing only every second day is sufficient to maintain a high level of gingival health, although careful brushing every third day is not.66,69 These studies were performed with dental students and they had a level of motivation and skill in tooth cleaning probably exceeding that of the ordinary patient. In the average patient, more frequent tooth cleaning is advisable, and a thorough cleaning twice a day is probably needed to keep the dental plaque at an acceptable level and keep the gingivae free from signs of bleeding on probing.
Susceptibility of oral bacteria to antibacterial photodynamic therapy

The oral cavity is one of most complex organs, consisting of various structures including teeth, tongue, and cheek tissue. More than 600 species or phylotypes reside in the oral cavity and many systemic diseases are related to the composition of the oral microbiota [1,2], which makes effective methods for managing the oral microbiome necessary to ensure the systemic health of a human body. Brushing is the most widely used method to control the bacteria in the oral cavity, but some areas such as pits and fissures of molars are difficult to clean [3]. Attempts to overcome this limitation have included modifying the shape of toothbrush bristles and using oral care products such as oral rinses and dental floss. In addition, many germs are present on the tongue, but the area near the throat is very difficult to clean due to the gag reflex [4]. The presence of dental structures with various shapes and sizes, such as implants, resins, brackets, and dentures, make the oral cavity complex. Microleakages occur at the interface between the teeth and these materials, which may cause diseases such as secondary caries and implant failure.

Power toothbrush is keeping updating all the time to improve its efficacy and user experience since the first introduction, which can be noticed in the development of its mechanism, brush head designs, modes of action, ergonomic features, etc. [4,5]. Generally, the power toothbrush family is constituted of battery-powered brush and rechargeable brush, with modes of action represented by oscillating-rotating (including rotating, counter-oscillating, back and forth and side to side) and sonic (including ultrasonic). As technology improved and people’s high demand for oral health, power toothbrushes show an increase in popularity for both the developed and developing countries. Nevertheless, its advantages as compared to the manual toothbrush is still not quite clear.

The Multinational Association of Supportive Care in Cancer (MASCC) recommends that oral care include routine tooth brushing and gargling with an oral solution once or more daily (Lalla et al., 2014) with lip care based on the use of lip moisturizer (UK Oral Mucositis Group (UKOMIC), 2015). Oral hygiene is a low-cost yet beneficial intervention to prevent mucositis (Mathur, Dhillon, & Kalra, 2012). Tooth brushing also helps to clear away plaque and debris, which contribute to local infection and mucositis, especially in immunocompromised patients. Thus, the MASCC recommends that patients brush their teeth twice a day for at least two minutes using a soft-bristled nylon toothbrush. These oral hygiene techniques are supported by the literature.
