Outcomes of Nonsurgical Retreatment and Endodontic Surgery: A Systematic Review

Two years before this systematic review was published, a Cochrane review by Del Fabbro et al. in 2007 had found similar results,18 in that only two randomised controlled trials exist2,11 that directly compared nonsurgical endodontic retreatment with endodontic surgery. Both articles had significant limitations. One was a study by Danin et al. in 1999,11 who reported one-year postoperative follow-up data of only 38 patients. The small sample size and short follow-up time in this study are generally felt to be insufficient to adequately assess long-term success.2 The other randomised study was by Kvist and Reit2 on 95 teeth, finding better short-term outcomes for surgery, but no difference at 48 months.

Regenerative endodontics comprises both vital and non-vital pulp treatments. Specifically, nonvital treatments include procedures to promote new vital tissue formation after necrosis following infection. The regenerative endodontic treatment is an alternative to a conventional endodontic treatment (Murray et al. 2007).

The ideal number of visits for root canal treatment is still being debated. A Japanese study has shown that the result of successful one-visit endodontic treatment is 86% after 40 months. While Sjogren showed an opposite result, the success rate of which is small (approximately 68%).1,2

Almost half of the included material made comparisons between their trained neural network model and other methods. Three papers on the periapical lesions compared the accuracy of their trained model with the performance of human experts [23,28,36]. Regarding the pulp cavity or root morphological studies, three papers compared their model with human experts [25,33,38]. Three compared their neural network model with non-AI technologies; a MATLAB-based method [40] Imaging Interaction Toolkit Workbench software [41] and a 3 D Slicer software [37]. One paper made comparisons between different U-net (a subgroup of CNN) models [35]. A paper on vertical root fracture compared the accuracy of its model on different types of radiographies (PA and CBCT) [26]. The paper on endodontic retreatment compared its model with dental clinicians [17].

Endodontic treatment is performed to retain teeth that would otherwise have to be extracted. In the field of clinical endodontics, the late 1990s was an important era for innovations and expansion of new technology. Innovation refers to any idea, practice, or object that is perceived as new by an individual, whereas technology is the application of scientific knowledge through machinery and equipment. Advances in technology are intended to improve endodontic treatment outcome. Some of the most pertinent introductions were the use of dental operating microscopes (DOM) for better visualization [1,2] electronic apex locators (EAL) to aid working length determination [3] and motor-driven file systems for instrumentation [4]. One technology that has evolved considerably due to innovations in metallurgy and material sciences is the motor-driven file systems. This has resulted in frequent introduction of new file systems. Newer file systems often claim to be more efficient, reliable and user-friendly than the current system. Thus, DPs have to make important decisions on whether the new system is indeed what it claims to be and integrate it into their clinical practice.
Periapical status transitions in teeth with posts versus without posts: a retrospective longitudinal radiographic study

In a review article, the proportion of teeth with post-retained restoration developed apical pathology was slightly lower than that of teeth without a post, but the difference was not statistically significant, and the heterogeneity of the studies reviewed substantial [30]. A recent observational study showed that placing a post increased the odds of periapical health for 85%, attributing this to the tight seal provided by posts and the bactericidal corrosive deposits sealing the canal interface in the cases of metallic posts and core restorations [12]. Differences in case selection may explain why the favourable results for posted teeth presented by Kielbassa and co-workers differed from the results of the current study [12]. Poor cementation of temporary crowns or temporary posts, temporary restorations over long periods can lead to coronal leakage especially in cases of delayed permanent restorations [31,32]. On the other hand, post preparation procedures may disrupt the apical seal and microorganisms may invade the post space between appointments, whereby the treatment itself becomes the risk factor for a negative endodontic outcome. Based on the results of this study, it seems reasonable to suggest shortening the time between the end of endodontic treatment and the final restorative treatment, in cases where prosthetic restorations are indicated.
