Infantile Colic

One randomized controlled trial was conducted in Denmark. One group of infants received spinal care for two weeks while the other was treated with the drug dimethicone. Between trial days four to seven, hours of crying were reduced by one hour in the dimethicone groups compared with 2.4 hours in the manipulation group. On days 8 through 11, crying was reduced by one hour for the dimethicone group, whereas crying in the manipulation group was reduced by 2.7 hours. From trial day five onward, the manipulation group did significantly better than the dimethicone group, which suggests that spinal manipulation is effective in relieving infantile colic.8

Dimethicone (polydimethyisiloxane) provides substantial conditioning effects to both hair and skin when properly delivered in appropriate quantities. Incorrectly formulated, dimethicone also has the potential to leave hair or skin feely tacky, greasy, and prone to attracting environmental dust and debris. The formulating challenge of delivering just the appropriate quantity of this category of materials is particularly acute for hair care products, since they are typically rinsed after application. If the dimethicone droplets are not sufficiently small, or adequately suspended, they will coalesce and settle out of the system. If they are emulsified by surfactants, they will, predominantly, be carried away with the rinse water and be of little or no value to the hair. Procter and Gamble researchers found a solution to this problem by suspending discrete dimethicone droplets with the aid of a natural gum.

Dimethicone is a mixture of siloxane polymers and can be used in creams at concentrations of up to 15% as a barrier agent. Dimethicone can prevent water loss across the skin and improve barrier thickness (Short et al., 2007). It also reduces melanin intensity, which may be an important part of photoaging, and improves dermatitis (Fowler, 2000). An extensive review of toxicity studies concluded that dimethicone and its variants were safe when used in cosmetic formulations (Nair, 2003).

Procedures were performed by expert endoscopists using pharyngeal local anesthetic spray Xylocaine (AstraZeneca, Luton, UK) or conscious sedation (midazolam/pethidine) according to patient preference. A black soft rubber hood (MB46, MAJ-1990, Olympus) was attached to the endoscope tip to allow a fixed 2 mm distance between gastric mucosa and gastroscope. All procedures were done with high definition and magnification gastroscopes (GIF-FQ260Z; Olympus Optical, Tokyo, Japan) and Lucera Elite CV290 video processor. The video images were viewed on a high definition video monitor (OEV-191H, Olympus). During the procedure the mucosa was washed with a mixture containing 100 ml of water mixed with 2 ml of acetylcysteine (200 mg/ml, Parvolex, Celltech, UK) and 0.5 ml (40 mg/ml) dimethicone (Infacol, Forrest Laboratories, UK). Detailed examination of the gastric mucosa was then carried out, in WLE and then NBI using both low magnification and magnified views. Still digital images were recorded in both WLE and NBI, with biopsies taken from the areas where the digital images were recorded. A minimum of 8 images were recorded for each patient. All still images were transferred to an external hard drive.

SAIB, Mw 856, specific gravity 1.146 kg L−1 at 25 °C, was obtained from Eastman Co., Ltd., St. Louis, MO, USA. Lakeshore® 7525 DLG 2 A (poly-d,l-lactide-co-glycolide, 75:25), Mw 14,000 Da, inherent viscosity 0.17 dL g−1 was purchased from SurModics Pharmaceuticals, Inc., Birmingham, AL, USA. RM (98%) was bought from Yancheng Langde Chemical Co., Ltd., Yancheng, China. Dimethicone, viscosity 350 cSt, was obtained from Dow Corning Corporation, Midland, MI, USA. Ethanol (EtOH), n-methylpyrrolidone (NMP), dichloromethane (CH2Cl2), and n-heptane were purchased from Sinopharm Chemical Reagent Co., Ltd., Shanghai, China. Span 80 was obtained from Nanjing Well Chemical Co., Ltd., Nanjing, China. Water was purified with a Milli-Q® (Bedford, MA, USA) filtration system, Millipore, USA. All other reagents and solvents were analytical grade.

Maternal background data, e.g. education level, smoking, and the intake of selective serotonin reuptake inhibitor or serotonin-norepinephrine reuptake inhibitors (SSRI/SNRI), were assessed using prenatal and postnatal self-report questionnaires (Karlsson et al., 2018). Information on the exposure to maternal prenatal distress, i.e. the case/control status, was based on standardized self-report questionnaires for symptoms of depression (Edinburgh Postnatal Depression Scale [EPDS]), overall anxiety (Symptom Checklist-90, anxiety scale [SCL-90]) and pregnancy-related anxiety symptoms (Pregnancy-Related Anxiety Questionnaire-Revised 2 [PRAQ-R2]) at gestational weeks (gwks) 14, 24, and 34. The exact criteria for becoming identified as a case or control have been comprehensively described earlier (Karlsson et al., 2018). Information about breastfeeding was collected from postnatal follow-up questionnaires and categorized, in accordance with Stewart et al. (Stewart et al., 2018), as never breastfed, breastfeeding ceased before 2.5 months of age, partial breastfeeding at the age of 2.5 months, and exclusive breastfeeding at the age of 2.5 months. In addition, information about infant probiotics, vitamin D, and dimeticone usage (any use vs. no use) were collected using the postnatal questionnaires. Information on maternal pre-pregnancy body mass index (BMI; kg/m2), duration of gestation, birth weight (g), mode of delivery (caesarian section vs. vaginal), as well as antibiotic intake of the infant during neonatal period were collected from National Birth Registry provided by the National Institute for Health and Welfare of Finland (www.thl.fi). The information on the mode of delivery was missing from two study subjects.
