Traumatic Carotid Sinus Reflex

One paper in the literature reports on a lethal carotid sinus syndrome that was probably due to an unusual complication after puncture of the carotid artery during positioning of a Shaldon catheter [9]. A 39-year-old woman was brought to hospital after acute renal failure following renal transplantation. Acute dialysis was planned: the right jugular vein should be punctured by a Shaldon catheter. However, the right common carotid artery was punctured. Bleeding into the soft tissue started immediately. After sudden and unexpected cardiac arrest, resuscitation was started but intubation failed. Stenosis of the trachea due to haematoma following the puncture was discussed as cause of death as well as thromboembolism and early septic process. Autopsy revealed an extensive retropharyngeal haematoma but no stenosis of the upper airways. Histological investigations showed extensive bleeding surrounding the sinus caroticum with subcapsular haemorrhages. The sudden onset of the cardiac arrest could have been initiated by a haemorrhage-induced irritation of the sinus caroticum.

The right and left common carotid arteries supply the head and neck. The anatomy of the arterial supply in rats is essentially similar to that of humans (Figure 4.1a,b). The internal and external carotid arteries are derived from their respective common carotid artery. The right common carotid artery is a continuation of the innominate artery. The left common carotid artery arises from the arch of the aorta (Figure 4.1).1–4

All assay procedures in this part were permitted by the Ethics Committee of Jiangxi Provincial People’s Hospital, the First Affiliated Hospital of Nanchang Medical College. Twenty male C57BL/6 J mice weighing 20–25 g and aged 5–7 weeks were purchased from Huafukang Biotechnology Co., Ltd. (Beijing, China), and housed in pathogen-free facilities with free access to food and water. These mice received MCAO surgery inferring to the reported method [15]. In brief, all mice were anesthetized using pentobarbital sodium (40 mg/kg) and connected to a ventilator (the breathing ratio was 1:1, the breathing frequency was 120 times each time, and the tidal volume was 0.15 mL) to sustain life. An arteriotomy was implemented after exposure of the right common carotid artery. Then, blood in this carotid artery was occluded using 4–0 nylon filaments. Meanwhile, sham operation (without MCAO operation) was carried out on control mice. Twenty-four hours later, the non-sham mice were treated with adeno-associated viral vector introduced with pCD5-ciR or circNUFIP2 sequence in the left lateral ventricle. After 48 h, these mice were euthanized by CO2 inhalation, and brains were harvested for the analysis of cerebral infarction, sensation, movement, reflex activity as well as balance.

A commercially available ultrasound machine was used for all of the ultrasound examinations. Carotid stiffness parameters were calculated with the application of the echo-tracking (eT) system incorporated in the Aloka ProSound Alpha 10 device. During the examination, the patient was in a relaxed, supine position while the ECG monitoring was performed. First, a clear longitudinal image of the walls of the right common carotid artery was acquired with a linear transducer at frequencies of 0 .7 MHz. Then, the eT gate for the measurement of arterial diameter changes across the cardiac cycle was positioned at the boundaries between the intima and media of the anterior and posterior wall of the Rt-CCA, 1–2 cm proximal to the bifurcation. Three to five beats were averaged to obtain a representative waveform. Finally, the value of blood pressure (measured on the brachial artery with the use of the certified OMRON sphygmomanometer) was entered into the system. Then, the computer automatically calculated the appropriate parameters, including the following: stiffness (β), One-point pulse-wave velocity β (PWVβ), Peterson’s elastic modulus (Ep), and arterial compliance (AC) (14, 15, 16).

Mice were anesthetized with ketamine 100 mg/kg and xylazine 5–10 mg/kg intraperitoneal, and a midline neck incision was made. The right common carotid artery was exposed, and the external carotid artery was isolated and ligated. A blunted 5–0 monofilament nylon suture was advanced into the internal carotid artery. The suture was advanced distal to the right anterior and middle cerebral artery bifurcation, where resistance was encountered, and then advanced 2 mm further to perforate at the right middle cerebral artery. The suture was withdrawn, allowing reperfusion and SAH. In sham-operated mice, the suture was advanced only until the point of resistance, thereby avoiding arterial perforation. After removal of the filament, SAH was confirmed by transcranial Doppler. The temperature was kept normal thermic with the heating pad. The skin was sutured, and mice were observed until recovery.
