The third lumbar vertebra is in the middle of the five lumbar vertebrae in the lower back portion of the spinal column. It also serves as an axis of movement of the lumbar region. In addition, the transverse processes of the third lumbar vertebra are thin and the longest among all the transverse processes of the lumbar vertebrae, but they are lacking transverse foramina. Its spinous process is very large and nearly rectangular in shape.41

Cerebrospinal fluid (CSF) is obtained by lumbar puncture, which involves insertion of a spinal needle between the fourth and fifth lumbar vertebrae (lower bones of the spine) into the space surrounding the spinal cord. A manometer is then attached to record the opening CSF pressure. This may reflect an increased intracranial pressure (ICP). The fluid may show xanthochromia (xanth = yellow, chrom = color), denoting some type of infection or possible drug interference.

The association between genitourinary surgery and the subsequent development of spinal osteomyelitis is well documented. The frequency of vertebral involvement in urinary tract infection varies from 10 to 70%. Prostatectomy or pyelolithotomy, and especially operations on pyonephrotic kidneys might result in vertebral osteomyelitis some months after operation. Spinal osteomyelitis affects the vascular metaphyseal region beneath the anterior longitudinal ligament, and then spreads across the disc via the nutrient vessels to involve adjacent vertebral bodies. Unlike malignant deposits, infective lesions destroy the disc space. Lumbar vertebrae are most often involved; T8-9 to L3-4 lesions have been reported. It occurs mainly in adults with a mean age between 60 and 70 years. Fever, back pain, and stiffness are the major findings in vertebral osteomyelitis. The first modifications seen by radiology do not appear before 2 to 8 weeks of evolution of the disease. Early technetium-99m bone scans are of great diagnostic help.158-162

The lumbar vertebrae are mainly composed of vertebral body and intervertebral discs. And as shown in Figure 1, vertebral body mainly includes transverse processes, spinous processes, upper/lower articular process, vertebral foramen, lamina and papillae. The intervertebral disc is composed of the upper and lower endplates, the nucleus pulposus and the annulus fibrosus [1]. The upper and lower endplates are located at the bottom and top of the annulus fibrosus and they are the cartilage structure, which can be considered as elastic body. The nucleus pulposus is located in the middle of the annulus fibrosus, which is the incompressible fluid structure and belongs to the viscoelastic body and can withstand greater pressure. The annulus fibrosus which belongs to the super elastic body, surrounds the nucleus pulposus and connects the adjacent vertebrae through the upper and lower endplates [2,3]. The vertebral body consists of cortical bone and cancellous bone and the material parameters of the two parts are different [4].

In this study, 104 patients with OVCF were analyzed retrospectively from February 15, 2014 to January 17, 2016. The age of the patients ranged from 57 to 89 years, with an average age of 69.3 ± 8.2 years (Table 1). All patients had a history of acute thoracic back pain, and stand-up and standing pain increased before operation. On examination, there were soft tissue tenderness and percussive pain around the thoracic and lumbar vertebrae. No patient had contraindications for aclasta, and were clearly diagnosed with OVCF by preoperative X-ray, CT scan, and MRI. All specimens were randomly assigned to control and experimental groups, each group having 52 patients. The specimen of the control group involved 63 vertebral bodies, including three cases of T7, three cases of T8, one case of T9, two cases of T10, four cases of T11, seven cases of T12, 14 cases of L1, 10 cases of L2, 13 cases of L3, five cases of L4, and one case of L5. The specimen of the experimental group involved 67 vertebral bodies, including one case of T6, two cases of T7, two cases of T9, three cases of T10, five cases of T11, nine cases of T12, 14 cases of L1, 11 cases of L2, 10 cases of L3, seven cases of L4, and three cases of L5.

Kidney cancer patients undergoing partial or radical nephrectomy were recruited at the Emory University Hospital. Blood and tumor tissue were collected at the time of surgery, and blood was collected from healthy donors. Patients’ magnetic resonance imaging (MRI) or computed tomography (CT) imaging within 3 months prior to surgery was obtained and the cross-sectional image at the third lumbar vertebra was identified. Body composition was analyzed via Slice-O-Matic Software V4.3 (Tomovision), as detailed in the supplemental methods.10 Samples were collected and this study was completed in accordance with an institutional review board approved protocol (IRB 00055316), and all patients provided informed consent.
