The following necessary precautions should be taken to prevent the spread of germs and nail infections. Nails should be kept short, and the undersides cleaned frequently with soap, water, and clean nail grooming materials before using them. Thoroughly scrub the bottom of nails with soap and water (or a nail brush) every time you wash your hands. When nail tools are shared among several people, as is the case in commercial nail salons, the materials must be sterilized before using them. Biting or chewing nails and cutting cuticles must be avoided as they act as barriers to prevent infection. Never rip or bite a hangnail but clip it with a sanitized nail trimmer.

It is also important to distinguish the magnitude of pain from how we react to it. Thus, it is useful to have two scales. The first one is, “On a scale of zero to ten where zero is complete comfort with no pain and ten is the most excruciating pain you have had or can think about, what are you feeling now?” The second one is, “On a scale of zero to ten, how much is this bothering you now?” You may have a pain of 7.3, and it only bothers you at a 3.1 level. Or, you might have a pain of 2.7 that bothers you at an 8.4 level. That is, for example, hangnails may have a low pain level but can be very bothersome. Also, it is not unusual for a person to experience a strong pain level, but one to which they have become somewhat inured. A surgeon I know told me that he uses these two scales all of the time (after he read about them in my book on healing language ( Battino, 2010)) and finds this quite useful.

Hangnail is a triangular-shaped overextension of the cuticle, which may separate from lateral or proximal nail folds, particularly during winter months or after prolonged exposure to water (Figures 37.4 and 37.5). Hangnails more frequently appear on the fingers than the toes. Superimposed infections in hangnail may cause swelling and redness of nail fold. It may be associated with a habit of cuticle biting. A mild to moderate hangnail can be treated conservatively by hot fomentation, trimming hangnail straight, and constant lubrication of fingertips with skin creams. Avoidance of repeated hand immersion in water is beneficial and preventive. Attempts at removal may cause pain and extension of tear into the dermis. Infected hangnails may require a short course of topical or oral antibiotic therapy.6

Disruption of the protective nail barrier allows for invasion of pathogens and subsequent infections, which can occur following trauma. Nail biting, finger sucking, aggressive manicuring and hangnail manipulation are common causes of minor trauma to the fingernails [47]. In the toenails, acute paronychia is often due to ingrown nails [50]. Onset of symptoms typically occurs within a week from the initial trauma [51]. Staphylococcus aureus is the most common pathogen; however, most infections involve a mix of aerobic and anaerobic bacteria [52,53].
