All patients undergo vascular imaging with magnetic resonance angiography or computerized tomographic angiography. Imaging is essential for planning this procedure because it is extremely important to know exactly where the primary perforator enters the flap. This point (or points) is designated by coordinates determined by the radiologist and confirmed by reviewing the imaging study. The coordinates of the perforators are established according to specific landmarks. The main perforators are identified in their vertical orientation by their distance caudal to the most inferior point of the infra-gluteal crease. Its distance from the midline establishes the perforator’s horizontal orientation. This gives the surgeon two points of reference. With the patient prone, these points can be located with a tape measure and marked. The marked points of the perforators are then confirmed with a Doppler signal.

The gluteal crease was marked; that was the uppermost extent of the excised tissue. The caudal extent of tissue resection was the middle transverse thigh fold. The marked saddlebag was pinched to simulate skin closure with no undermining. The surgical markings were tapered upwards over the anterolateral region of the thigh in order to obtain a tension-free wound closure while achieving a pleasing contour with a fold-targeted excision. On the medial aspect, a pinch test was used to assess the amount of skin excess. The pinching maneuver outlined the direction to connect the anterior to posterior markings. An evaluation of an ideal buttocks should consider the relationship with the hip, the legs, and the lower back. A ratio of 0.65 between the hip and the waist should be proposed in the anterior view. In the lateral view, a prominent gluteal crease should be used [17, 18]. We should keep in mind that liposuction or any plastic surgery procedure is not a substitute for a healthy lifestyle. We know that there is some benefit to metabolism, but if we do not discuss thisbenefit with patients, they are at risk to suffer from chronic metabolic diseases [19].
