This is related to over-expression of the enzyme choline kinase, which is responsible for phosphocholine synthesis [308, 397]. It also reflects altered membrane choline phospholipid metabolism. The major steps in choline metabolism in mammalian cells are through the cytosine diphosphate (CDP)-choline pathway [308, 309, 400]. It should be noted that therein are several proton MR visible compounds, namely: choline (3.21 ppm), PCho (3.22 ppm) and GPC (3.23 ppm), thereby emphasizing the clinico-biological importance of analyzing the relationship among these closely overlapping resonances. On the other hand, by summing these three metabolites as “total choline”, as is currently done with in vivo MRS, substantial information for breast cancer diagnostics is missed.

Choline was firstly labeled with 11C. After intravenous injection, 11C-choline is rapidly cleared from blood flow and then internalized into the cells by both facilitative transport mechanisms and passive diffusion; then, once in the cytoplasm, choline is phosphorylated to phosphocholine (PC) and subsequently, after a series of other metabolic steps, incorporated in the cell membrane [51]. Labeled choline has been successfully applied for the PET imaging of prostate cancer and other tumors [52–54].
