Biomarkers for nutrition assessment in children with CKD are individualized to the stage or dialytic therapy (Table 20.10). Hypoalbuminemia is a marker of mortality in this patient population and remains an important biomarker to track. However, it is not indicative of the presence of malnutrition but rather sensitive to fluid status, proteinuria, or inflammation. Prealbumin should also be used with caution as an assessment of nutrition status in children with renal dysfunction. Those on dialytic therapy are known to have inherent inflammation. As prealbumin is a negative acute phase reactant, this could also be reflective of the ongoing inflammation related to their care and disease.

Poor linear growth has been observed in some patients with PKU, and this has been thought to result from protein insufficiency. The level of prealbumin has been found [68] to correlate well with protein adequacy in this disease, and the threshold level is 20 mg/dL. Linear growth can be expected to be impaired in patients with levels lower than 20 mg/dL.

Serum prealbumin is also referred to as transthyretin. It is synthesized in the liver and is involved in the transport of thyroid hormones. In assessing malnutrition, the concentration of serum prealbumin below 10 mg/100 mL is significant (Beck and Rosenthal 2002). It is an important biomarker in children and elderly patients. (Takeda et al. 2003). It has a shorter half-life period (two days) compared to albumin (Takeda et al. 2003). It is a more sensitive biomarker for protein depletion than albumin. Prealbumin concentration is highly sensitive to any alteration in the concentration of protein in the body as in protein-energy malnutrition (Takeda et al. 2003).

The multiple linear regression model at 12 mo, after surgery is shown in Table 4. Hemoglobin was positively related to scores on the physical functioning (β = 4.5; p = 0.012) and hair loss (β = 3.4; p = 0.034). Prealbumin was negatively associated with constipation symptoms (β= −7.8; p = 0.042) and hair loss (β= −6.1; p = 0.001). In addition, hair loss symptoms also was positively related to NLR and PLR (p < 0.05, see Table 5). The AGR level had a negative relation to scores on fatigue, insomnia, dry mouth and body image (p < 0.05). Patients with a high SII were more likely to have worse role functioning (β= −15.5; p = 0.011). PNI was positively associated with financial problems (β = 0.6; p = 0.048). AAPR was negatively correlated with scores on the emotional functioning scales, and it was negatively associated with pain, anxiety, and hair loss (p < 0.05).

The measurement of serum nutritional markers, including albumin, prealbumin, and transferrin, was incorporated into the initial diagnostic workup of the patients. The patient’s blood was collected with a vacutainer by venipuncture of the forearm to obtain about 10 ml. The collected blood was centrifuged at 4 °C for 10 mins,. Serum in the upper part of the tube was collected for analysis of albumin, transferrin, and prealbumin levels by the Sandwich enzyme-linked immunosorbent assay on an Immage 800 protein chemistry analyzer (Beckman Coulter, USA), using Beckman Coulter’s serum kit. The default settings of the normal limits for albumin, transferrin, and prealbumin in our hospital were 3.5 g/dL, 200 mg/dL, and 17 mg/dL, respectively.

Prealbumin, also known as transthyretin, has been routinely used in clinical practice which could efficiently reflect the nutritional status of patients. Although it is main synthesized by the liver, prealbumin is less affected by liver diseases than the other serum proteins and not significantly affected by blood transfusion (13,14). Thus, prealbumin may be more suitable for liver cancer patients’ nutritional status evaluation. Up to date, several studies have reported the prognostic significance of prealbumin in liver cancer, but the results were not conclusive. To address this question, current study aimed to assess the prognostic value of prealbumin in liver cancer.
