Sugiura et al.83 reported CU to di(2-ethylhexyl)phthalate in the dotted PVC grip of cotton gloves. They subsequently reported CU to polyethylene gloves in another patient.84 This patient had positive scratch test reactions to octadecanoic acid methyl ester, 2,6-di-tert-butylphenol, di(2-ethylhexyl)phthalate, cis-13-docosenoic acid amide, and octadecyl 3-(3,5-di-tert-butyl-4-hydroxy-phenyl) propionate; these chemicals are used as antioxidants and lubricants in polyethylene. Both patients were brothers who had chronic urticaria, which worsened when they wore gloves at work. Cow's milk casein is used as a stabilizer in glove manufacturing and can cause CU syndrome in individuals with cow's milk allergy.85 CU was also reported to be caused by polyvinylchloride (PVC) gloves in a patient who developed three episodes of pruritus of the hands and wrists, each within minutes of donning the gloves. Her symptoms of localized pruritus progressed to generalized pruritus, edema of the lips and tongue, and dyspnea. This patient showed early patch test reactions to pieces of the PVC glove, but no other early or delayed patch test reaction to any of the additives usually found in PVC gloves.86

The urinary metabolome has also been evaluated in n = 1 preterm neonate with invasive fungal infection using GC-MS; biomarkers as N-glycine, D-serine, L-threonine, D-glucose, and maltose increased in septic urine. On the contrary, citric acid, hexadecanoic acid, and octadecanoic acid decreased instead of n = 13 healthy preterm neonates. D-serine was significantly higher in patients with fungal sepsis, gradually decreasing after a targeted therapy. Amino acid could increase due to proteolysis, in response to the increase in energy demand related to sepsis. This occurs to face the hypermetabolic state in response to acute stress, requiring energy from the use of all the available substrates. A unique urine metabolic profile characterized the neonate with fungal sepsis compared to urine of controls, and it was also possible to evaluate the efficacy of therapy in improving patient health (23).
Fungal Lipids

Among Basidiomycotina, fatty acid records are most commonly derived from the readily-collected fruit-bodies of macrofungi of the Hymenomycetes and Gasteromycetes and may differ from the composition of vegetative mycelium, which has been studied in relatively few species. Oleic and linoleic acids are the predominant components in both classes. Several species have shown a higher degree of unsaturation in lipids of fruit-bodies than in mycelium.69,70 Hymenomycetes which have been studied intensively in mycelial stages include the cultivated mushroom Agaricus bisporus.71 The lipid composition of the smut and rust fungi, biotrophic phytopathogenic microfungi included in the Basidiomycotina, has been studied in relation to their interactions with host tissue.8,9,72 The smut fungi are readily grown saprophytically and appear to resemble Hymenomycetes in their fatty acid composition, whereas the rust fungi, which are more difficult to culture as mycelium, have been mainly studied in spore stages and show lipid characteristics distinct from other Basidiomycetes.72 Although pathogenicity to humans and animals has not usually been associated with Basidiomycotina, instances of infection have been recorded, particularly in compromised patients, e.g. Coprinus cinereus and Schizophyllum commune (Hymenomycetes) and two genera of smut fungi, Ustilago and Filobasidiella spp.2 The presence of cis-9,10-epoxy-octadecanoic acid as a major fatty acid is characteristic of rust spores, accompanied in some species by threo-8,10-dihydroxystearic acid, which is also found in some Claviceps species.8,11,72,131 A rapid decrease of the former was noted by Sock,72 during germination of Puccinia graminis urediniospores to a low level in axenic mycelium with a concomitant increase in α-linolenic acid.

In order to dig out the compounds, MCSE was analyzed by GC-MS. GC-MS analysis indicated that MCSE contains 25 chemical components with an elution time of 27.02–52.59 mins, (Figure 1 and Table 1). Chemical composition records are based on their relative retention times and mass spectra compared to authentic samples and/or NST/NBS and Wiley libraries spectra (Table 1). Most of the compounds identified were Octadecanoic acid and its derivatives.
