Sasaki et al.103 examined the effects of the potent hepatic arterial constrictor angiotensin II104,105 following intrahepatic arterial administration in nine patients with liver cancer or metastases using the extremely short-lived radioisotope 81mKr (half-life 13s). They found that after the start of the infusion radioactivity increased in the tumor region in comparison with normal liver, which decreased. Relative blood flow to the tumor region continued to increase to a maximal tumor/liver ratio of 3.30. The effect was maximal at 100 sec with a slow return to normal, a duration of action that is suited to bolus administration of microspheres rather than long-term infusion therapy.

Ask yourself ‘Will this patient really he able to cope with the demands of looking after an infusion pump?’. This is important. Home infusion therapy, particularly where an electromechanical infusion pump is used, requires patients or their immediate carers to take an active role in their treatment. If they are unwilling or unable to do so, other treatment options should be explored. Normally they should also have access to a telephone in case problems arise out of hours.

Infusion therapy is the administration of medication dripped through a needle or catheter in the patient's bloodstream. Home infusion has been proven to be a safe and effective alternative to inpatient care for many diseases, if done correctly.8

Infusion therapy for patients is predominately performed through the connection of a fluid medication bag and administration tubing, all attached to a patient’s indwelling IV catheter. Continuous infusions of medications or solutions require the patient to maintain a constant connection to the IV. The normal patient activity of bed transfers, transportation to different departments, movements in bed, frequent trips to the bathroom, and even patient confusion can all result in excessive pulling on IV tubing. As the tubing becomes stretched, the tension is transferred to the catheter and dressing with subsequent loosening of securement and accidental removal of the IV catheter. Forceful removal of the catheter and dressing can cause injury to the patient, skin damage, and loss of venous access. Prevention of accidental catheter dislodgement through the insertion of a safety releasing valve can relieve the tension on the tubing and catheter to protect the patient and facilitate uninterrupted treatment. The Infusion Nurses Society (INS) Standards recommend using catheter protection devices for specific patient populations, including pediatric, elderly, and those with cognitive dysfunction at risk for the catheter being accidentally dislodged or removed [5]. A responsibility to patients exists to provide safety devices, when available, to prevent these types of complications.

This model-based investigation of risk factors for poor HRQoL among adolescents with IBD has several limitations. First, latent variables of adherence behaviors, psychosocial problems, and HRQoL were created using adolescent self- and parent-report measures. Although this approach reduces error variance and increases power, it treats disagreements between youth and parents “error” when in fact the diverging perspectives might be of substantive interest.73,74 Additional research is needed to examine differences in adolescent and caregiver reports of psychosocial problems, adherence behaviors, and HRQoL. Second, a majority of adolescents in the present study (47%) received intravenous biologic (ie, Infliximab) treatment. It is important to note that these adolescents are more likely to be adherent to their medical regimen since they receive infusion therapy on a fixed schedule (ie, every 6 to 8 weeks) in clinic.

For patients with deterioration of oxygenation, rapid imaging changes and excessive inflammatory response, glucocorticoids can be used as appropriate. Experts from the Chinese Thoracic Society developed a consensus statement that patients can be treated with glucocorticoids when they meet the following four conditions at the same time: (1) adult (age ≥18 years old); (2) diagnosed with new coronavirus infection by PCR or serum antibodies; (3) symptoms (including fever, cough or other related infection symptoms) occur within 10 days, radiographically confirmed pneumonia and rapid progress; and (4) blood oxygen saturation (SpO2) ≤93% or shortness of breath (breath rate ≥30 breaths/min) or oxygenation index ≤300 mmHg [121]. For patients with secondary infection, intestinal microecological regulators and broad-spectrum antibacterial drugs can be used. Immunoglobulin infusion therapy can also be used to improve patient immunity [122]. It is recommended that pregnant women with severe COVID-19 infection end their pregnancy as soon as possible. In addition, anxiety and fear are often present in severe and critical cases, and the psychiatric service must have a specific approach for dealing with these problems including hospital resource management, mental health intervention, and guidance for family members [123].
