It was in 1929, in the midst of this new age of neuropharmacological exploration, that Gordon Alles patented his new discovery, amphetamine.10 The use of amphetamine for the treatment of depression was first reported in 1936 by Guttman11 and Meyerson.12 It helped for some, but side effects sometimes included anxiety and agitation. Initial well-being was sometimes followed by worse depression, tolerance, and addiction. Methylphenidate, pipradol, and deanol were other stimulant drugs used in the 1950s for monotherapy of MDD prior to the predominant use of the soon to follow MAO (monoamine oxidase) inhibitors and tricyclics antidepressants.13 The discovery and use of the “modern” antidepressants, MAO inhibitors and tricyclics in the 1950s, and the later discovered selective serotonin reuptake inhibitors (SSRIs), will be discussed at length in the following chapter.

Fourteen RCTs were included with a total of 364 participants; these were mostly conducted within hospital settings in the USA (2 studies recruited outpatients and one study was undertaken in each of the following countries: Japan, Canada, Australia, Iran, Switzerland and Germany). All studies compared a cholinergic drug against a placebo for the following treatments while the participants remained on their antipsychotic medication: n = 6 trials used Deanol, n = 4 Lecithin and n = 1 trial was found for each of the following: Meclofenoxate hydrochloride, Galantamine, Rivastigmine, Donepezil.
