Godfrey’s cordial from the 18th century was another popular remedy, containing opium, treacle, and sassafras, used for treating rheumatic pains. This preparation was freely available in most grocery shops without prescription or restriction, despite many people becoming addicted. As late as the 20th century, opium was viewed as a legitimate substance, like tobacco and tea, but by 1920, the use of opiates was prohibited, and could only be prescribed by doctors if patients demonstrated a legitimate medical need.

A variety of preparations can be used but the most common are polyethelene glycol and mannitol. All are not absorbed and have high osmolarity, ensuring retention in the bowel and good distension. The T2 relaxation time of the preparations is similar to that of water. The preparations have an unpleasant taste and can be modified with fruit cordial. Transit times are 40–50 minutes and there can be a mild cathartic effect. Children who may not be able to drink the preparations can be reasonably successfully imaged using a water-only preparation of 500–1000 ml.

Jared also felt that he could not form a connection with many users because of the manner in which they would approach him. “I just feel like on … It was more about men just wanting to have sex with a Black man. It was real nice [talking to you].’ [laughs] And then I’d probably stop replying.” In this instance, a conversation was halted immediately after a user admitted to only wanting Jared for sex, which prevents users who might otherwise share commonalities from bonding. Later in the interview, Jared spoke about how he and his friends commiserate over feeling fetishized: Most of my friends do have similar experiences actually. They don’t really like it for the same reasons. It’s just … a lot of this sounds like we are used as a fetish, and who wants to be just used for a fetish? Can we actually get a connection? Can we at least be cordial? Be friends? And you just want to get your rocks off because I’m a Black man. I’m not cool with that.

Cordial doctor-patient communication showed positive moderating effect of the relationship between patient satisfaction and treatment adherence. This suggests that cordial communication increases patient satisfaction and is pivotal in the biomedical treatment process, in that it does not only improve the patient’s healthcare quality experience, but also enables the patient to understand his or her illness, risk profile as well as treatment options and benefits [54]. Other studies have profiled the importance of cordial communication in medical encounters to include information exchange and assimilation, fostering therapeutic relationship and enabling patient trust with healthcare providers’ knowledge, skill, and commitment [25,55]. These aspects of communication considerably improve patient satisfaction and treatment adherence. Appointment scheduling of IBS also fosters discrete interaction at the interface between the physician and the patient, particularly when the interval between successive patients is reasonable. We think that this inherent benefit of APS allows for cordial communication to thrive thereby promoting strong patient affinity to the provider and increasing the chances of treatment adherence.

Participants visited the laboratory on three separate occasions: 1) anthropometric measures followed by full experimental familiarisation, 2–3) experimental trials. The experimental familiarisation was a separate visit whereby participants completed the full experimental trial (see below) of cycling, however, without ingestion of any solution. The experimental trials were completed using a randomised crossover design, with these trials separated by 7 days, conducted at the same time of day (±1 h), and the day of and prior to any experimental trial marked by abstinence from alcohol and exercise and only habitual caffeine use (as abstinence would confound from withdrawal effects). Additionally, participants were asked to replicate their diet during the first experimental visit for the subsequent trial to ensure a similar metabolic state. Experimental trials consisted of the treatment (NaHCO3, BIC) and a placebo (NaCl, PLA) dissolved in fruit-flavoured cordial. All trials were completed on the same cycle ergometer (Monark, Varberg, Sweden), where participants’ set-up (e.g. seat/handle bar height etc.) was customised and replicated for each subsequent visit.
