Urine Multistix include stabilized, diazotized 2,4-dichloraniline, which reacts with bilirubin to form azobilirubin. The test will detect about 3 µmol/L of bilirubin. Drugs, such as large doses of chlorpromazine, may give false-positive results.

Reaction of rubins with diazo reagents leads to formation of two dipyrrolic azo derivatives horn the parent tetrapyrrole (Chapter 1, Figure 11). For nomenclature used to denote these dipyrroles the reader is referred to Chapter 1. The name azobilirubin is misleading and has been replaced by azopyrromethene (Azpm). Alternatively, the equally acceptable trivial name, azodipyrrole, has also been used. Two isomeric azopyrromethenes, Azpm-12 and Azpm-8, are derived from bilirubin-IXα. They have been denoted previously as the “isovinyl”, or exo-vinyl isomer (Azpm-12) and the “vinyl”, or endo-vinyl isomer (Azpm-8) of azopyrromethene. From a bilirubin-IXα diglucuronide molecule one would obtain an exo- and endo-vinyl isomer of azopyrromethene glucuronide, corresponding with Azpm-12(GlcU) and Azpm-8(GlcU), respectively.

The classic method, known as the Jendrassik-Groff diazo method, estimates total serum bilirubin concentration from the reaction between bilirubin and diazotized sulfanilic acid in the presence of accelerating agents like acetate, caffeine, and benzoate to give red azobilirubin, which is quantified spectrophotometrically [72,73]. Without an accelerator, the diazo method provides an estimate of direct bilirubin, which includes conjugated and delta bilirubin. Delta bilirubin can be considered an interference and may give rise to spuriously elevated bilirubin measurements [74].

The bilirubin was estimated by using commercially available test kit of the standard method. In brief, bilirubin reacts with diazotized sulfanilic acid to produce a colored azobilirubin compound. The intensity of the color formed is directly proportional to the amount of bilirubin present in the sample measured by spectrophotometer at 630 nm (Simmons 1968).
