Different extracts obtained from the fresh leaves of Allium ursinum were tested in vitro on human platelet aggregation. The results showed a significant inhibitory activity of the ethanol extract on ADP-induced aggregation. The mechanism of action was similar to that of a reference drug Clopidogrel (Hiyasat, Sabha et al. 2009). It was suggested that the active compounds exerting antiaggregatory effect are 1,2-di-O-α-linolenoyl-3-O-β-D-galactopyranosyl-sn-glycerol (DLGG) and β-sitosterol 3-O-β-D-glucopyranoside (Sabha, Hiyasat et al. 2012).

Despite recognition as an ingredient in potent traditional medicines, the history of Veratrum spp. is rife with accidental poisoning. Veratrum intoxication is most often due to inadvertent consumption as a result of misidentification during foraging [11–19]. Cases of V. viride poisoning are reported to have occurred due to mistaking the plant for Symplocarpus foetidus (skunk cabbage), Phytolacca americana (pokeweed), Allium ampeloprasum (wild onion), and Allium tricoccum (ramps) [12,13,15]. Likewise, V. album, a species most prominently found in northern Eurasia and localized regions of Alaska, is reported to have been misidentified as Allium ursinum (wild onion) and Gentiana lutea (yellow gentian) [16,19]. Most cases of poisoning manifest with symptoms including diarrhea, nausea, vomiting, and a Bezold-Jarisch reflex (bradycardia, hypopnea, and hypotension). Patients with mild symptoms typically respond well to symptomatic and supportive treatments with intravenous fluids and antiemetics, but those with symptomatic bradycardia or hypotension may require treatment with atropine or vasopressors [11–19].

One of the largest genera of monocots is the genus Allium L., which belongs to the family Amaryllidaceae (Govaerts et al. 2021). Allium ursinum L. (known as wild garlic, ramson, bear’s garlic, etc.) is a bulbous perennial plant widely distributed from temperate Europe to the Caucasus (Tutin 1957). The bulb is naked with a few parallel fibres at the base. Leaves are basal with a distinct petiole, narrowly elliptical to narrowly ovate, and acute. The solitary scape is two-angled or trigonous. The flowers are arranged in semispherical umbel-like inflorescences with two-valved spathes that are shorter than the flower pedicels. The perianth segments of the flowers are white, lanceolate, and acute. The stamens are shorter than the perianth segments. The ovary is deeply lobed with an entire stigma, and the fruit is in a capsule (Tutin 1957). A. ursinum inhabits woodlands and grows best in habitats that are constantly moist and well-illuminated (Tutin 1957). The plant material (A. ursinum leaves) were collected in the Gaj locality (Bosnia and Herzegovina, Laktaši municipality, 44° 55′ 18″ N, 17° 19′ 01″ E, 120 m a.s.l.).

A 74-year old Caucasian female (71.5 kg body weight, BMI 25) with no prior medical history was admitted for unintentional colchicine overdose after ingestion of a pizza covered with colchicum autumnale leafs 9 h before hospital admission. She had mistaken colchicum autumnale for allium ursinum. After stabilization in a district hospital, she was transferred to our tertiary care facility. Activated charcoal was administered.
