Phantosmia occurs in a wide variety of conditions from temporal lobe epilepsy (Luciano, Alper, and Nadkarni 2011), schizophrenia (Meats 1988), and LSD intoxication (Preston, O’Neal, and Talaga 2010) to sinusitis (Hirsch 2004) and Parkinson’s disease (Meats 1988; Hirsch 2004, 2009b). Even weather changes are associated with phantosmia (Aiello and Hirsch 2013). All these usually occur in association with reduced or absent ability to smell. Those who suffer from phantosmia often display psychological distress and depression (Hirsch and Trannel 1996).

Hyperosmia is heightened sense of smell. Dysosmia (or parosmia) is abnormally perceived sense of smell. Phantosmia (olfactory hallucination) is smell perception in the absence of an odor stimulus. Olfactory agnosia is the inability to recognize olfactory sensory information in the setting of intact olfactory function.

Malingering of isolated phantosmia, or hallucinated smells, in the absence of loss, would be highly unusual and would most likely occur as part of another syndrome, which includes phantosmia as one of its components. Phantosmia occurs in a wide variety of conditions from temporal lobe epilepsy (Luciano, Alper, and Nadkarni, 2011), schizophrenia (Meats, 1988), LSD intoxication (Preston, O’Neal, and Talga, 2010), to sinusitis (Hirsch, 2004) and Parkinson’s disease (Meats, 1988a; Hirsch, 2004; Hirsch, 2009b). Even weather changes are associated with phantosmia (Aiello and Hirsch, 2013). But, all these usually occur in association with reduced or absent ability to smell. Those who suffer from phantosmia often display psychological distress and depression (Hirsch and Trannel, 1996). Detection of malingering of phantosmia, as would detection of the malingering of any hallucination (whether it be visual entopias, or even formed visual scenes) would rely on inconsistency of presentation in history (changing from one visit to the next or even during the same visit) and absence of evidence of any other chemosensory, otorhinologic, or neurologic dysfunction.

In previous interviews and questionnaires, we collected the following information: basic characteristics such as sex, age, history of systemic diseases (such as hypertension, diabetes, cardiovascular and cerebrovascular diseases), history of nasal and oral diseases, history of trauma, vaccination times, history of smoking and alcohol drinking, and diet preference; associated symptoms after SARS-CoV-2 infection, such as fever, fatigue, nasal congestion and runny nose, sore throat, cough, headache, muscle or joint pain; drugs administration during the SARS-CoV-2 infection; status of anxiety and depression (by HADS); and changes of smell and taste, such as the visual assessment scale (VAS) of smell and taste and the Taste Strips (Burghart Messtechnik, Germany) to evaluate different types of flavors affected after SARS-CoV-2 infection. The alteration types of smell or taste (such as parosmia, phantosmia, phantogeusia, and parageusia) were also noted.

The patient had an uneventful recovery; phantosmia and dysgeusia resolved on Keppra. She was referred to neuro-oncology and it was agreed that she would undergo a full course of chemotherapy given her overall good health and excellent performance status despite her age.
