Odontogenic Tumors

Ameloblastic fibrosarcoma consists of a benign epithelial component intermingled within a hypercellular malignant mesenchymal stroma and is considered as the malignant counterpart of ameloblastic fibroma. The tumor shows thin, elongated islands of odontogenic epithelium set in a myxoid mesenchymal stroma under the microscope.

Hybrid-CGCG lesions with CODF mostly depicted a cortex at the periphery (11 of 14, 78.6%) and favoured a radiolucent internal structure (11 of 14, 78.6%). Cortical expansion was described in 10 out of 14 hybrid-CGCG lesions with CODF, and all were found expansile. CODF comprises 0.1% of all odontogenic tumours. It is composed of odontogenic epithelium and fibrous stroma [41]. Radiographically presents as unilocular or multilocular radiolucency, and in rare cases, appears mixed [41]. It is radiographically indistinguishable from hybrid-CGCG lesions with CODF and mimics ameloblastoma, odontogenic keratocyst, odontogenic myxoma, and ameloblastic fibroma [41].

Enucleation and curettage are the main treatment of AFO as this odontogenic tumor is considered as a non-aggressive lesion and unlike ameloblastic fibroma its recurrence and malignant transformation is not usual [6]. AFO recurrence rate is remarkably lower than ameloblastic fibroma (seven percent versus 18-35% respectively). Ameloblastic fibroma recurrences may reflect malignant transformation of this tumor; therefore it requires marginal resection and frequent follow-up [7]. In contrast, AFO malignant transformation is not expected and has been recorded only as case reports (two cases by Howell and Burkes) in the literature [8].
