A variety of evidence-based diets had been demonstrated to assist in healthy weight loss. These include the Mediterranean diet, DASH diet, and the Healthy U.S. Style Eating Pattern. It has also been demonstrated that differences in macronutrient composition of weight loss plan (e.g., low fat versus low carb) do not yield different results in studies lasting longer than one year. In both the DPP and the Look AHEAD Trial, individuals were able to keep off greater than 90% of the weight that they lost over three to four years. In the National Weight Control Registry which has over 10,000 individuals who have lost greater than 50 pounds and have kept it off for over one year, regular attention to monitoring nutrition as well as regular physical activity is the key component of how these individuals have been able to maintain weight loss.
Maintenance of Weight Loss

The NWCR is an observational cohort study of self-reported, long-term successful weight loss maintenance (4,37). Started in 1994, the NWCR is tracking more than 5000 individuals who have lost at least 13.6 kg (30 lb) and kept it off for at least 1 year. Registry members have lost an average of 30 kg (66 lb) and kept it off for 5.5 years. The weight losses have ranged from 13.6 to 136 kg (30–300 lb); the duration of successful weight loss has ranged from 1 year to 66 years. Some individuals lost the weight rapidly, whereas others lost weight very slowly, over as many as 14 years. The demographics show 80% of persons in the NWCR are female and 20% are male. The average woman is 45 years of age and currently weighs 66 kg (145 lb), whereas the average man is 49 years of age and currently weighs 86 kg (190 lb). About half of the participants lost the weight on their own, and the other half lost weight with the help of some type of program. Most of the registry participants had childhood onset obesity. Forty-six percent report being overweight by 11 years of age, 28.3% report being overweight by 12 to 18 years of age, and 28.3% report being overweight by 18 years of age. Forty-six percent report one parent was overweight, whereas 26.8% report both parents were overweight. Ninety-eight percent of these participants report they modified their food intake in some way to lose weight; 94% increased their physical activity, with the most frequently reported form of activity being walking.

Energy expenditure, mainly in the form of physical activity, is at least as important, and possibly more so, than reduced energy intake in helping to control body weight. Energy expenditure is the somewhat neglected child of obesity research because it is more difficult to measure and assess than energy intake, but its importance is finally beginning to be more widely appreciated (Wendel-Vos et al. 2007). Participants in the National Weight Control Registry have made rigorous daily exercise a regular part of their successful maintenance regimen, strong proof of its value in controlling obesity (Phelan et al. 2003). A one-year study of participants with extreme obesity clearly demonstrated that physical activity contributed significantly to weight loss and improvement of cardiac health (Goodpaster et al. 2010).

In the current study, NWQ is defined as the remote and very remote (MM6 and 7) [32,33] region serviced by the North West Hospital and Health Service (NWHHS). NWHHS catchment area is 300,000 km2 with an estimated population of 28,000, of which 23.3% identify as Aboriginal and/or Torres Strait Islander [34]. The main industries in the area include mining, agriculture and tourism [35]. People experiencing stroke in NWQ who are deemed eligible for rehabilitation are referred to the nearest tertiary facility, located ∼900km by road from Mount Isa. North West Community Rehabilitation (NWCR), based in Mt Isa, offers a small ongoing public service, in addition to existing public and private allied health services within Mt Isa. Across the NWQ region outside of Mt Isa, allied health services are available intermittently. Difficulties recruiting and retaining a skilled health workforce to provide services coupled with a lower population health status contribute to the inequity of service accessibility in the region [36] compared to metropolitan areas. Use of telehealth to access services is growing in the region [36].

Maintenance of weight loss must remain a life-long effort as the weight loss itself triggers a number of changes in hormones that fuel hunger as well as a reduction in energy expenditure that are long-lasting. The National Weight Control Registry has identified practices for achieving and maintaining weight loss among a group of registrants who have lost at least 13.6 kg and sustained that for over a year. These include eating breakfast each day, high levels of volitional physical activity (1 h per day), reduced fat intake, self-monitoring of dietary intake and activity, consumption of low or no-calorie sweeteners, regular monitoring of weight and limiting television viewing [54]. Figure 1 summarises the approach to managing overweight and obesity in women.
